Cheshire West and Chester is seeking to commission a service to provide targeted support to vulnerable children and young people aged 0 - 18 (25 if the young person has Special Needs) years, but predominantly for ages of 8 and 18 years, in order to support those who have multiple and complex needs at Level 3 on the continuum of need.
