The subject of the order is the supply of gift cards (hereinafter referred to as cards) for employees of the Grand Theater - National Opera in the form of electronic bearer prepaid cards confirmed with a signature without a PIN in the total number of 985 pcs.

1. The subject of the order is the supply of gift cards (hereinafter referred to as "cards") for employees of the Teatr Wielki - Polish National Opera in the form of electronic bearer prepaid cards confirmed with a signature without a PIN in the following quantity:

- for 89 employees with a value of PLN 1,400.00 for PLN 124,600.00,

- for 430 employees with a value of: PLN 1,200.00 for an amount of PLN 516,000.00,

- for 241 employees with a value of: PLN 1,000.00 for PLN 241,000.00,

- for 225 employees with a value of: PLN 900.00 for PLN 202,500.00.

A total of 985 cards with a total value of PLN 1,084 100.00 (say: one million eighty-four thousand one hundred, 00/100).
