1. The subject of the contract is the provision of health services. The provision of health services will include the provision of:
a) as part of the basic contract - for NFEP&WM employees and pensioners (former NFEP&WM employees).
b) under the right of option – for family members of NFEP&WM employees and family members of pensioners (former NFEP&WM employees).
The basic contract provides that the benefit will include: employees, with an average monthly employment determined at the level of 802 people, Members of the Management Board - 5 people, as well as pensioners, with the average monthly number of pensioners at the level of 125 people.
The option right provides for an average monthly number of family packages at the level of 371 pieces and the number of people covered by the senior package - 91.
A detailed description of the subject of the contract is presented in Annex 4 to the SWZ.

The contracting authority will have the right to exercise the so-called option right within 36 months from the date of signing the contract, by providing the Contractor with an appropriate statement.

The estimated value of the contract in question is set at PLN 8,946,502.92 net, including the estimated value of the contract covered by the option right is set at PLN 3,027,681.24 net.
