The subject of the contract is the development of a full, complete, multi-branch design and cost estimation documentation for the creation of the Central Operating Block with the Central Sterilization Plant in Tomaszów Centrum Zdrowia Sp. z o.o. in Tomaszów Mazowiecki at ul. John Paul II 35, 97-200 Tomaszów Maz., according to the concept constituting an annex to the SWZ (hereinafter also referred to as: the concept), along with the author's supervision.
The subject of the contract is the development of a full, complete, multi-branch design and cost estimation documentation for the creation of the Central Operating Block with the Central Sterilization Plant in Tomaszów Centrum Zdrowia Sp. z o.o. in Tomaszów Mazowiecki at ul. John Paul II 35, 97-200 Tomaszów Maz., according to the concept constituting an annex to the SWZ (hereinafter also referred to as: the concept), along with the author's supervision.
In the scope of the contract, the Contractor is obliged to: transfer to the Ordering Party the author's economic rights to the completed study, without territorial or time limitations, in all fields of exploitation known at the time of conclusion of the contract, in the scope of the entire study and any part thereof, and to allow the Ordering Party to exercise dependent copyrights and authorize the Ordering Party to commission third parties to exercise dependent copyrights; undertake not to exercise moral rights to the developed documentation, and consent to the exercise by the Ordering Party of moral rights to the developed documentation.
The documentation should include the extension of Tomaszów Centrum Zdrowia Sp. z o.o. with a new two-storey, non-basement building including: a central sterilization room, a central operating block consisting of 5 operating rooms, including one hybrid, 5-bed recovery room; located in the courtyard space between the existing buildings B1, B2, B3, C, functionally connected to the elevator hall in building B3 on all designed floors, which will ensure a functional connection of the new facility with the existing ones.
Detailed characteristics of the investment plan for which the ordered design and cost estimation documentation is to be created is included in the concept constituting an annex to the SWZ.
The ordering party requires that the design and cost estimation documentation for the facility takes into account energy-saving solutions and optimal renewable energy sources (to the extent possible to obtain, the resulting facility is to be energy-efficient and use renewable energy sources).
The concept constituting an annex to the SWZ defines the minimum scope that should be included in the design and cost estimation documentation, which will be subject to detail and extension at the stage of developing the design and cost estimation documentation: in accordance with the requirements resulting from the regulations, the needs of the Ordering Party, medical standards, functionality, on the basis of other studies, expert opinions carried out, inventories, analyzes, balances, demands and conclusions from them. Resulting.
After the written approval of the Purchaser's representative, changes in relation to the concept of the m.in resulting from the regulations or if they are more favorable for future functionality or result from an equivalent solution or result from the inspection, inventory and it turns out that the solution adopted in the concept is impossible or insufficient; or other, but accepted by the Ordering Party or by the Ordering Party submitted.
