The subject of the contract are the supply of medical oxygen and other items of the contract, the lease of cryogenic tanks, cylinders and equipment for the supply of nitric oxide and the service of inspection of the internal installation of medical gases for the Fryderyk Chopin Clinical Provincial Hospital No. 1 in Rzeszów, Szopena 2 Street and Rycerska 2 Street, including:

a) supply of medicinal products: liquid medical oxygen, compressed medical oxygen, medical nitrous oxide, nitrous oxide, anesthetic gas, medical compressed air and supplies of medical devices: medical liquid nitrogen, medical carbon dioxide – Annex 1 A to the SWZ;

b) supply of medical devices: disposable exhaust valve with mouthpiece for anesthetic gas – Annex 1 B to the SWZ;

c) lease of cryogenic tanks, cylinders, equipment for the supply of nitric oxide and the service of inspection of the internal installation of medical gases – Annex 1 C to the SWZ.

1. The subject of the contract are the supply of medical oxygen and other objects of the contract, the lease of cryogenic tanks, cylinders and equipment for the supply of nitric oxide and the service of inspection of the internal installation of medical gases for the Fryderyk Chopin Clinical Provincial Hospital No. 1 in Rzeszów, 2 Szopena Street and Rycerska 2 Street, including:

a) supply of medicinal products: liquid medical oxygen, compressed medical oxygen, medical nitrous oxide, nitrous oxide, anesthetic gas, medical compressed air and supplies of medical devices: medical liquid nitrogen, medical carbon dioxide – Annex 1 A to the SWZ;

b) supply of medical devices: disposable exhaust valve with mouthpiece for anesthetic gas – Annex 1 B to the SWZ;

c) lease of cryogenic tanks, cylinders, equipment for the supply of nitric oxide and the service of inspection of the internal installation of medical gases – Annex 1 C to the SWZ.

2. A description of the ordering party's needs, including the requirements and quantities of deliveries and services in the period covered by the order, is included in the Price and Offer Forms (Annexes No. 1 A, 1 B, 1 C to SWZ), which are an integral part of swz. A detailed description of the subject of the contract is presented in Appendix No. 2 to the SWZ. General conditions for the lease of equipment for storage, evaporation, mixing and operation of medicinal and medical gases are included in Annex 3 to the SWZ.

3. The scope of the contract includes the supply of medicinal products and medical devices, the lease of cryogenic tanks, cylinders, equipment for the supply of nitric oxide and the service of inspection of the internal installation of medical gases in the quantities specified in the Price and Offer Forms, which are an integral part of the SWZ, for a period of 24 months.

4. The Contracting Authority shall not allow the submission of equivalent tenders.

6. Deadline for the execution of the contract: for 24 months, counting from 30.12.2022. (i.e. after the end of the current contract by 29.12.2022) or from the date of conclusion of the contract in the event of an extension of the tendering procedure.

The Ordering Party allows the possibility of changing the term of validity of the contract concluded with the Contractor in the event of failure to implement the entire subject of the contract during its duration. In this case, the amendment of the contract requires the signing of an annex, except that the Ordering Party unilaterally notifies the Contractor about the proposed extension of the contract before the expiry of the period for which it was concluded (does not apply to the lease).
