Supply of medical equipment for UCK - 264/PN/2023

1. The subject of the contract is the supply of medical equipment with technical parameters specified in Appendices No. 4a-4ł to the Terms of Reference, hereinafter referred to as the "equipment", together with the installation and training of the Contracting Authority's employees.

2. A detailed description of the subject matter of the contract is included in section IV of the Terms of Reference.

Infusion pump set

1. The subject of the contract is the supply of medical equipment with the technical parameters specified in Appendix 4a to the Terms of Reference, hereinafter referred to as the "equipment", together with the installation and training of the Contracting Authority's employees.

2. The subject matter of this order shall include the following parts:

Part 1 – A set of infusion pumps with the technical characteristics specified in Annex 4a to the ToS,

3. The above-mentioned medical equipment shall meet the limit parameters set out in Annex 4a to the Terms of Reference, respectively. Failure to meet at least one of the thresholds set out in Annex 4a to the Terms of Reference, respectively, will result in the rejection of the tender.

4. The equipment listed in point 2 shall be delivered together with the equipment necessary for its proper functioning.

5. The delivery of the equipment referred to in point 2 shall take place at the place indicated by the Ordering Party in Gdańsk, at 17 Smoluchowskiego Street, at the Contractor's expense.

6. The Contractor shall train the Employer's employees in the basics of medical equipment operation within the time limit specified in the draft contract.

7. The Contractor shall be obliged to guarantee that the delivered medical equipment is brand new, complete, of a high standard both in terms of quality and functionality, as well as free from material, construction and legal defects, approved for use in public health care facilities.

8. The Contractor shall be obliged to provide, within the delivery date of medical equipment, copies of documents confirmed as being in conformity with the original, which show that the offered assortment has been admitted to trading and use in the Republic of Poland.

9. The offered medical equipment must be brand new, manufactured no earlier than within the deadlines specified in Appendix 4a of the Terms of Reference.

10. Where terms indicating the type, standard, trade marks or origin of the subject-matter of the contract are used in the documentation relating to the procedure in question, they shall be read together with the words or equivalent. Proper names are exemplary, specify the class of the product and serve to establish a standard - they do not indicate a specific product or a specific manufacturer. The Contractor, offering an object equivalent to the one described in the specification, is obliged to maintain equivalence in terms of technical, functional, dimensional and quality parameters, which must be at a level not lower than the parameters indicated by the Ordering Party. The burden of proving that the offered articles are equivalent to the requirements specified by the Ordering Party shall be borne by the tenderer.

Ultrasound (1 pcs.)

1. The subject of the contract is the supply of medical equipment with the technical parameters specified in Appendix No. 4j to the Terms of Reference, hereinafter referred to as the "equipment", together with the installation and training of the Contracting Authority's employees.

2. The subject matter of this order shall include the following parts:

Par t 10 — Paediatric bed with the technical characteristics set out in Annex 4j to the ToC,

3. The above-mentioned medical equipment shall meet the limits set out in Annex 4j to the Terms of Reference, respectively. Failure to meet at least one of the thresholds set out in Annex 4j to the Terms of Reference, respectively, will result in rejection of the tender.

4. The equipment listed in point 2 shall be delivered together with the equipment necessary for its proper functioning.

5. The delivery of the equipment referred to in point 2 shall take place at the place indicated by the Ordering Party in Gdańsk, at 17 Smoluchowskiego Street, at the Contractor's expense.

8. The Contractor shall be obliged to provide, within the delivery date of medical equipment, copies of documents confirmed as being in conformity with the original, which show that the offered assortment has been admitted to trading and use in the Republic of Poland.

9. The offered medical equipment must be brand new, manufactured no earlier than within the deadlines indicated in Appendix 4h of the Terms of Reference.

10. Where terms indicating the type, standard, trade marks or origin of the subject-matter of the contract are used in the documentation relating to the procedure in question, they shall be read together with the words or equivalent. Proper names are exemplary, specify the class of the product and serve to establish a standard - they do not indicate a specific product or a specific manufacturer. The Contractor, offering an object equivalent to the one described in the specification, is obliged to maintain equivalence in terms of technical, functional, dimensional and quality parameters, which must be at a level not lower than the parameters indicated by the Ordering Party. The burden of proving that the offered articles are equivalent to the requirements specified by the Ordering Party shall be borne by the tenderer.

Paediatric bed (5 pcs)

1. The subject of the contract is the supply of medical equipment with the technical parameters specified in Appendix No. 4i to the Terms of Reference, hereinafter referred to as the "equipment", together with the installation and training of the Contracting Authority's employees.

2. The subject matter of this order shall include the following parts:

Par t 9 — Paediatric bed with the technical characteristics set out in Annex 4i to the ToS,

3. The above-mentioned medical equipment shall comply with the limits set out in Annex 4i to the ToS respectively. Failure to meet at least one of the thresholds set out in Annex 4i to the Terms of Reference, respectively, will result in the rejection of the tender.

4. The equipment listed in point 2 shall be delivered together with the equipment necessary for its proper functioning.

5. The delivery of the equipment referred to in point 2 shall take place at the place indicated by the Ordering Party in Gdańsk, at 17 Smoluchowskiego Street, at the Contractor's expense.

6. The Contractor shall train the Employer's employees in the basics of medical equipment operation within the time limit specified in the draft contract.

7. The Contractor shall be obliged to guarantee that the delivered medical equipment is brand new, complete, of a high standard both in terms of quality and functionality, as well as free from material, construction and legal defects, approved for use in public health care facilities.
