1. The subject of the contract is group life and health insurance for employees of State Administration Units and members of their families.
2. Detailed requirements regarding the subject of the contract are specified in the Description of the subject of the contract, which is included in Annex 2 to the SWZ.
3. The detailed manner of implementation of the subject of the contract is specified in the Draft provisions of the contract - Annex 3 to the SWZ.

3. The contracting authority does not provide for the award of contracts referred to in Article 214(1)(7) of the PPL.
4. The Contracting Authority does not provide for an electronic auction.
5. The contracting authority shall not allow the submission of variant bids within the meaning of Article 92(1) of the PPL.
6. The Contracting Authority shall not allow the submission of partial tenders.
7. The contracting authority does not provide for the conclusion of a framework agreement referred to in Articles 311 to 315 of the PPL.
8. The contracting authority provides for the application of the procedure referred to in Article 139(1) of the PPL.
9. The Ordering Party does not provide for reimbursement of the costs of participation
10. The Contracting Authority does not allow the submission of offers in the form of electronic catalogues or attaching electronic catalogues to the offer.
11. The contracting authority may cancel the procurement procedure if the funds that the Contracting Authority intended to allocate to finance all or part of the contract have not been granted to it, and the possibility of cancelling the procedure on this basis has been provided for in the contract notice.
12. The Contractor joining the proceedings is obliged to pay a bid bond in the amount of: PLN 130,000.00 (in words: one hundred and thirty thousand zlotys 00/100).
