The subject of the order is: successive delivery of orthopedic implants to SP ZOZ Siemiatycze, in the quantities and assortment specified in the assortment form in terms of quantity and price, application to the successive reported needs (orders) of the Ordering Party, described in detail in Annex 2 packages from 1 to 5 of the Terms of Reference, constituting an integral part of it, and then an integral part of the delivery contract constituting Annex No. 1 to the contract. 2. A detailed description of the subject of the contract, a list of the required limit parameters covered by the delivery listed in paragraph 1 shall include an assortment and quantity form with a description, constitute Annex 2 to the Terms of Reference in detail to the offer, constituting an integral part of it, and then an integral part of the delivery contract.
