The subject of the contract is the Contractor's conducting a training and activation cycle for 5 groups of convicts serving a prison sentence in the Remand Center in Piotrków Trybunalski, Prison in Łowicz, Prison in Sieradz, Prison in Garbalin and Prison No. 1 in Łódź organized as part of the project entitled: "Improving the professional qualifications of prisoners in order to return to the labor market after completing their imprisonment" financed from the Operational Program Knowledge Education Development 2014-2020, consisting of two trainings:

1) vocational training "Carer for the elderly and disabled": theoretical part (22 teaching hours), practical part (40 teaching hours). As part of theoretical classes, part of vocational training is OHS training with 2 teaching hours and premedical first aid training with 8 teaching hours;

2) competence training entitled "Professional activation - ways of looking for a job" - 20 teaching hours.

Task No. 1

One teaching hour is 45 minutes.

Classes will take place at penitentiary units on business days, from Monday to Friday between 8.30 - 15.30 (with an hour lunch break), and with the prior consent of the head of the unit - also on Saturdays.

a) Group 1: Remand Center in Piotrków Trybunalski - Piotrków Trybunalski, ul. Wronia 76/90;

b) Group 2: Prison in Łowicz - Łowicz, ul. Wiejska 3,

c) Group 3: Prison in Sieradz - Sieradz, ul. Orzechowa 5,

d) Group 2: Prison No. 1 in Łódź - Łódź, ul. Beskidzka 54,

e) Group 3: Prison in Garbalin - 99-100 Łęczyca, Garbalin

The Contractor, in consultation with the manager of each of the above-mentioned units, will determine an individual schedule of classes, which he will then forward to the Employer - before starting the contract in a given unit. Changes to the schedule, both in terms of modifying the hours of conducting classes and conducting classes on Saturday, can only take place after consultation with the head of the unit, of which the Contractor shall immediately inform the Employer.

Number of participants, change of participant:

The training will take place for 5 groups - a total of a maximum of 60 convicts (12 convicts in each group). The Ordering Party undertakes to have a reserve list of course participants (minimum 2 people per unit), enabling easy introduction of a new person in place of the person resigning or excluded from participation in the course. A change of personnel during the cycle may take place no later than 20% of the hours of vocational training so that the person can complete the whole cycle and receive relevant certificates.

Implementation of the cycle, responsibility:

The contractor will undertake to carry out activities related to conducting trainings with due diligence and on the principles arising from the regulation of the Ministry of National Education of 18.8.2017 on lifelong learning in non-school forms (hereinafter the ordinance of the Ministry of National Education).

The trainings will start on the date agreed with the ordering party, immediately after signing the contract, and will end by November 30, 2019.

The Employer shall not be liable for damages caused by the Contractor during the implementation of the training and activation cycle.

The contractor declares that he meets the requirements set out in the regulation of the Ministry of National Education for continuing education in out-of-school forms.

Documentation:

The contractor will keep documentation for each cycle in accordance with the requirements of § 21 of the Ministry of National Education, i.e. curriculum, class diary, records of issued certificates and a report on the completion of the assessment.

Prisoners will confirm their participation in the classes with their own signature on the attendance list, submitted with a blue or green pen.

Within 2 business days from the date of signing the contract, but before the start of the first training, the Contractor will present to the Employer for approval
