The subject of the contract is the purchase of an angiograph along with the adaptation of rooms necessary for the functioning of the Hemodynamics Laboratory at the Cardiology Department at the Provincial Specialist Hospital No. 2 in Jastrzębie-Zdrój - task 1. A detailed description of the subject of the contract is specified in the Assortment and Price Form (technical and functional parameters) constituting Annex 1 to the SWZ. Additional conditions for the implementation of the subject of the contract are specified in the Model contract constituting Annex 5 to the SWZ.
