4.1. The subject of the contract is the purchase of services of private medical care for employees of the University of Lodz.
The private medical care service includes group medical care for employees of the Ordering Party and their family members in the form of a medical subscription in the following packages:
1) individual package - dedicated to the employee,
2) partnership package - dedicated to an employee and one person from: spouse / partner / child (regardless of age),
3) family package – dedicated to the employee and spouse / partner and children regardless of their number (up to the age of 18, and if they are educated until the completion of education, no longer than until the age of 25).
The order also includes the option "adult child". Each of the packages indicated above will be available in three variants: basic, optimal and extended.
4.2. A detailed description of the subject of the contract is attached as Annex 1a to the SWZ.

This option will be available to an employee using a partner package or family package and applies to a child after the age limit specified in point 4.1. point 3). An employee using a partner or family package may, for an additional fee (price for the adult child option), register an "adult child" as an additional person over the persons specified in point 4.1. points 2) and 3) above to the package he or she uses himself.
Each of the packages indicated above will be available in three variants: basic, optimal and extended.
