The subject of the order is group life insurance for employees and family members of employees employed in organizational units of the Łęczyński Poviat.
The Employer does not guarantee that all employed employees and family members of Employer's employees will join group life insurance.
During the contract period, there may be changes in the number of insured persons.
If an employee or a family member of an employee resigns from insurance, the Employer is not obliged to pay insurance premiums for them.

The Employer requires employment on the basis of an employment contract by the Contractor or a subcontractor of persons performing activities during the performance of the contract consisting in booking the impact of the insurance premium for the period resulting from the contract and payment of compensation for damage to the Employer.
The Employer has the right to inspect the Contractor in terms of compliance with the requirements set out in point 1. At the request of the Employer, within a time limit indicated by the Employer not less than 5 working days, the Contractor undertakes to submit for inspection a statement confirming that the employees referred to in paragraph 1 are employed under an employment contract.
Failure to submit the documents referred to in item 2 by the Contractor shall be treated as failure to fulfill the obligation specified in the ToR and Art. 29 paragraph 3a of the Act.
The contracting authority provides for the possibility of concluding an order during the term of the contract consisting in the repetition of services similar to the basic contract constituting 20% of the basic contract. The contract will be awarded for services in accordance with the subject of the basic contract in the mode of single-source procurement, after meeting the conditions of art. 67 section 1 point 6 of the Act.
