The subject of the contract is: catering – preparation and delivery of all-day food for patients to the ward stoves of the 116th Military Hospital with the SPZOZ Outpatient Clinic in Opole, taking into account diets and calorific value:
a) daily (breakfast, lunch, dinner) for the departments of general surgery and ENT - 14 beds, internal medicine - 26 beds, care and treatment facility (ZOL) - 40 beds, rehabilitation and rheumatology - 20 beds,
b) on weekdays (lunch type two breakfast) for the daily psychogeriatric ward – 24 people.
2. Detailed description of the item of the order:
2.1 Preparation and delivery to the hospital's ward cookers of all-day meals:
a) providing meals three times a day prepared according to a fixed menu at the following times:
- Breakfast from 7.15 to 7.45
- lunch from 12.00 to 12.30,
- dinner from 16.15 to 16.45
- diabetic supplement - second dinner (1/2 portion of dinner) from 16.15 to 16.45
It is allowed to provide dinner with the delivery of breakfast or lunch, provided that the freshness of the meal is maintained and conditions are secured in accordance with applicable regulations,
(b) meals must be delivered daily all days of the year to the hospital's ward cookers;
c) meals for patients of ZOL should be adapted for the elderly (soft, shredded, etc.),
2.2 Preparation and delivery of meals - lunch (e.g. in the form of a sandwich, soup, stew, salads from cooked ingredients) for the daily psychogeriatric ward (for patients over 60 years of age):
a) it is required to deliver a meal, the so-called lunch once a day by 11.00 a.m. on weekdays,
b) The Ordering Party allows the delivery of lunch together with the delivery of breakfast, except that the lunch should be properly secured and stored in conditions ensuring the freshness of the meal until 11.00, in accordance with applicable regulations.
2.3 Provision by the Contractor of equipment for storing and transporting meals on its own.
2.4 The Contractor shall ensure the quality of meals and their transport as well as the storage of samples in accordance with the applicable hazard analysis and critical control points system called the "HACCP System".
2.5 Hot meals should be delivered in thermoses, ensuring the temperature of the food in accordance with the requirements, i.e.: the minimum temperature of the soup should be 75ºC, the second course 65ºC, liquids 80ºC, and the maximum temperature of cold products (salads, pastes, cheeses, etc.) should be 15ºC.
The Ordering Party requires that all-day nutrition be based on diets, according to the Ordering Party's menus, constituting Annex 9 to the SWZ:
(a) basic diet
b) easily digestible / ulcer / hepatic diet

(e) niskobiałkowa diet
f) diet bogatobiałkowa
g) diet bezmleczna
h) bezglutenowa diet

Number of meals in accordance with Annex 2 to the SWZ
The subject of the contract will be carried out in accordance with the description of the subject of the contract, the model contract (Annex No. 5 to the SWZ) and Annex No. 9 to the SWZ.
