Short Term Home Support is an intensive service for adults to support hospital discharge and/or prevent hospital admission. The service provides a personalised service to help people retain or regain their independence. The ethos is around enabling/re-abling, rather than ‘doing’ for service users to support the individual achieve their outcomes and goals.
