1. The subject of this order is the gradual delivery of the range intended for stabilizing patients in radiation therapy for the Oncology Center Maria Skłodowska-Curie, Branch in Gliwice, hereinafter referred to as the Employer.
2. Description of the subject of the contract and the conditions of the contract are included in the forms:
a) assortment and price specification - Annex 2 to the ToR;
b) model contract - Annex 4 to the ToR.
3. In order to assess the quality, the Purchaser requires submission of a free sample of the offered assortment - 1 item for item 1. The sample must comply with the assortment offered in the assortment and price specification.
4. The Awarding Entity does not envisage concluding a framework agreement, establishing a dynamic purchasing system, electronic auction and supplementary orders.

2. The awarding entity in this tender procedure will use the procedure set out in Art. 24aa of the PPL Act.
3. Employer, pursuant to art. 147 section 1 of the Public Procurement Law, requires the Contractor whose offer was considered the most advantageous to provide a performance bond in the amount of 1% of the gross price of the offer.
4. The Awarding Entity, prior to awarding the order, to confirm that the range offered meets the requirements set out by the Awarding Entity will call the Contractor whose offer has been rated the highest to be submitted within a designated period, not shorter than 10 days, valid on the date of submission of the CE certificate or declaration of conformity ( confirming the CE marking).
5. The documents referred to in para. 4 should be submitted in the original in the form of an electronic document or in an electronic copy of the document certified as being true to the original.
6. The documents referred to in par. 4, prepared in a foreign language, should be submitted together with a translation into Polish.
