1. The item of the order was divided into two packages:
Package No. 1
a. The preparation in the kitchen of the Contractor all day board for patients of the District Hospital in Mikołów at 2 Waryński Street and for patients of the Department of Care - Care (ZPO) in Łaziski Medium at Staszica 4 Street
b. delivery of meals to the Nutrition Department in the hospital and the reception point of meals in the ZPO by means of transport of the Contractor (in one or two cars)
c. discharge of containers with meals from the car and transporting the Contractor's trolley to the indicated location
d. Reception of empty containers after meals and their washing and disinfection in the premises of the Contractor
Package No. 2
Preparation and delivery of breakfasts (approx. 10 breakfasts throughout the term of the contract) as described in Annex 2, point. II

2. In connection with the specifics of the object of the contract, the Contracting Authority pursuant to Article 95 (1) of the Law of Public Procurement requires that persons performing the performance of the contract indicated by the Contractor or Subcontractor on the basis of an employment contract throughout the term of the contract. In the event of termination of the employment of the persons referred to above before the end of the date of implementation of the contract, the Contractor or Subcontractor shall be obliged to direct another person employed in the required form immediately to the place.
3. At the latest on the date of signing the contract, the Contractor shall submit to the Contractor or Subcontractor a name list of persons employed by the Contractor or Subcontractor on the basis of an employment contract
4. Each invoice the Contractor shall be joined by a statement of the contractor or subcontractor on employment under the contract of employment of persons performing the service to the Contracting Authority. This declaration should include, in particular, the precise determination of the entity making the statement, the date of the declaration, the indication that the services provided to the Contracting Authority are carried out by persons employed under the employment contract, with the indication of the number of those persons and the signature of the person empowered to make a declaration on behalf of the contractor or subcontractor;
