The subject of the contract is to grant and maintain the state of readiness to provide the Contracting Authority with health care services for the Contracting Authority and its employees and managerial staff throughout the term of the Agreement.
The subject of the contract is to grant and maintain the state of readiness to provide the Contracting Authority with the following health services throughout the term of the Agreement, for the benefit of the Contracting Authority and its employees and managerial staff:
- occupational health care services – in accordance with applicable law, in particular: the Labour Code, the Regulation of the Minister of Health and Social Care of 30 May 1996 on conducting medical examinations of employees, the scope of preventive health care for employees and medical certificates issued for the purposes provided for in the Labour Code and the Act of 27 June 1997 on the Occupational Medicine Service,
- health services not included in the scope of occupational medicine.
At the same time, the Contracting Authority indicates that the condition for the performance of the contract for health services is an additional commitment of the Contractor to:
- providing health services to family members of the Contracting Authority's employees.
The full cost of subscriptions for the employee's family members is borne by the employee who registers the family member for medical care. Payment for this subscription is made by the Ordering Party, and the relevant amount is deducted from the salary of the employee registering the family member for medical care.
- providing health services to pensioners and their family members on the basis of separate agreements concluded individually with these persons on terms not worse than those described in detail in the model agreement 23DOZZ315. In order to fulfil this obligation, the Contractor is obliged to indicate in the content of the offer the subscription fees for the types of subscriptions listed in the contract template 23DOZZ315. However, the Contractor's health benefits for pensioners and their family members are not the subject of the contract, and thus the Contracting Authority's obligation. The actual number of benefits will depend on the individual decisions of the Contracting Authority's pensioners made after receiving information about the prices for each of the packages.
The cost of subscriptions for the benefit of the pensioner and the pensioner's family members shall be borne in full by the pensioner, who decides to purchase this subscription from the Contractor and sign a separate agreement with him. The condition for the provision of the service under the above-mentioned subscription is the conclusion of a separate agreement by the Contractor with the pensioner. The terms and conditions of the agreement signed between the Contractor and the pensioner may not be worse than the terms and conditions specified in the Agreement and appendices. Payment for these subscriptions is made directly by the pensioner in cash, by payment card or bank transfer to the Contractor's account.
Health services will be provided in the company's own facilities or in those cooperating with the Contractor, as well as in the on-site outpatient clinic at the Żerań CHP Plant (15 Modlińska Street) in Warsaw, at the Siekierki CHP Plant (30 Augustówka Street) in Warsaw, at the on-site outpatient clinic at the Kawęczyn Heat and Power Plant (180 Chełmżyńska Street) in Warsaw and at the doctor's office at the Pruszków CHP Plant (1 Waryńskiego Street) in Pruszków. The renovation of the company's outpatient clinics and the doctor's office at the Pruszków CHP Plant is the responsibility of the Ordering Party.
