The scope of the service includes:
a) basic order - preparation of project documentation - construction and detailed design together with obtaining a decision on permission to carry out a road investment for the task "Expansion of Voivodeship Road No. 479 - ul. Uniejowska in Sieradz - II Stage "
b) option - performing author's supervision over the implementation of the task entitled "Expansion of Voivodeship Road No. 479 - ul. Uniejowska in Sieradz - II Stage ”.
1. The scope of the service includes:

b) option - performing author's supervision over the implementation of the task entitled "Expansion of Voivodeship Road No. 479 - ul. Uniejowska in Sieradz - II Stage ”.
BASIC ORDER
The scope of project documentation includes the extension of voivodship road No. 479 ul. Uniejowska in Sieradz, i.e. from approx. Km 27 + 560 to approx. Km 29 + 017 with particular emphasis on sidewalks, bicycle paths, bus bays and drainage.
The scope of the study does not include the extension of the intersection with DK 83, while the intersection should be included in the design of the target traffic organization.
As part of the contract, multi-branch design documentation should be made to obtain a permit decision for the implementation of the road investment and to perform all construction works necessary for the proper implementation of the expansion of voivodship road No. 479 - ul. Uniejowska in Sieradz - II Stage.
The order includes the following items:

Land development project,
Architectural and construction design for road expansion,
Executive design of the road extension,
Architectural and construction design for expansion of road drainage,
Executive design for the expansion of road drainage,
Architectural and construction design for the engineering industry,
Wykon Executive design of the engineering industry,
Architectural and construction design for reconstruction of the teletechnical network,
Executive design of the teletechnical network reconstruction,
Architectural and construction design for the reconstruction of the power grid,
Executive design of the reconstruction of the power grid,
Architectural and construction design of road lighting construction,
Awczy Executive design of the road lighting construction,
Architectural and construction design for reconstruction of the water supply network,
Awczy Executive design of the reconstruction of the water supply network,
Architectural and construction design for reconstruction of the sanitary sewage system,
Executive design of the reconstruction of the sanitary sewage system,
Establishing geotechnical foundation conditions,
Project Information Card (KIP) together with the necessary attachments to the application for the issuing of the Environmental Protection Department,
Environmental impact report - if required,
Design of traffic organization for the duration of the works,
Target traffic organization design,
Information on safety and health protection,
Water law report with obtaining water law permit, water law approvals,
Materials for obtaining an opinion on the application for a decision on permission to implement a road investment,
Materials for the application for a decision on the permission for the implementation of the road investment,
Obtaining all other required arrangements and decisions, provisions in accordance with the detailed requirements, including the relevant protocol from the coordination meeting
STWiORB - for each industry based on STWiORB ZDW.
A detailed bill of quantities for each industry,
Bid valuation form compliant with STWiORB,
Investment cost estimate.
OPTION
Employer pursuant to art. 34 section 5 Public Procurement Law provides for the right of options.
2. The Awarding Entity provides for the possibility of awarding contracts referred to in art. 67 section 1 point 6 Public Procurement Law up to the amount of PLN 97 250.00 net The contracting authority may award contracts involving the repetition of similar services consistent with the subject of the basic contract when it has the necessary financial resources.
