Description:

Purchase and delivery of contrast agents for medical entities of the Podkarpackie Voivodeship.

1. The subject of the contract is the purchase and supply of contrast agents to the entities listed in

section I.1 of the announcement (except for the entity: Podkarpackie Medical Center in Rzeszów SPZOZ).

A detailed description of the subject of the contract is included in Annex 4 to the Specification of Conditions

orders.

2. The complete offer shall be made (obligatory):

1. The offer is prepared in accordance with the template of Annex 1 to the SWZ. Offer form.

2. Completed Offer and price form in accordance with the template constituting Annex 4 to the SWZ.

Other documents/statements that the Contractor is obliged to submit together with

with the offer:

3. Declaration in the form of an ESD according to Appendix No. 2 to the SWZ;

4. Statement, according to Annex 7 to the SWZ – if applicable;

5. Declaration according to Annex 8 to the SWZ.

6. Power of attorney or other document confirming the authorization to represent the contractor;

7. The evidence in question - the statement contained in the form Offer form - Annex 1 to the SWZ
