NHS Wiltshire CCG wishes to procure community-based Ophthalmology Services for service users registered with a Wiltshire GP. The model is for a Provider to have multiple clinical treatment sites/hubs utilising an Ophthalmologist at its core for decision making, but Optometry delivered care, diagnostics and treatment. The scope includes:
— IOP Referral Refinement,
— Cataract Referral Refinement,
— Community Eye Service,
— OHT and Suspect / Stable Glaucoma Community Management,
— Cataract Community Follow-up,
— Low Vision Assessment.
Contracts will be for an initial term of 24 months from 1.6.2018, with a possible extension of any period up to a further 12 months, as defined at the discretion of the Commissioner.
Wiltshire CCG has an annual budget available of 502 000 GBP (incl. VAT) based on the indicative annual volumes indicated in the Tender Documentation.

It is envisaged that the service will assess, diagnose and treat a number of conditions and provide a Referral Refinement service to ensure appropriate referrals are made to secondary care. The service will assist in the management of long term conditions and supported self-management for patients with such conditions. The model is for a Provider to have multiple clinical treatment sites/hubs across the county utilising an Ophthalmologist at its core for decision making, but Optometry delivered care, diagnostics and treatment. The scope of the service will include but not be limited to:
— IOP Referral Refinement,
— Cataract Referral Refinement,
— Community Eye Service,
— OHT and Suspect / Stable Glaucoma Community Management,
— Cataract Community Follow-up,
— Low Vision Assessment.
It is envisaged that a broader range of conditions or treatments may be included in the service during the contract life.
This is a new service which aims to support improving patient care and experience through offering a range of services in a more localised setting.
Contracts will be for an initial term of 24 months from 1.6.2018, with a possible extension of any period up to a further 12 months, as defined and at the discretion of the Commissioner.
Wiltshire CCG has an annual budget available of 502 000 GBP (incl. VAT) based on the indicative annual volumes indicated in the Tender Documentation. This value is indicative and no volume guarantee will be provided. The total volumes may result in the value of the agreed contract being below or above the estimated annual budget. The estimated total value reflects potential changes to the volumes that may be achieved through the contract life.
