The subject of the Order is:
1. delivery of instruments for surgical procedures in hybrid technology to the Contracting Authority's registered office (Cracow, 1 Złota Jesień Estate), in accordance with the description and requirements contained in Appendix 1A to the Contracting Authority. Specification.
2. rental of hybrid generators – 2 pcs. (apparatus), as described and requirements in Annex I to
No. 1B to the SWZ.
Other terms and conditions of the contract are set out in the draft agreement constituting Appendices 3A and 3B to the Terms of Reference.
The subject of the Order is:
1. delivery of instruments for surgical procedures in hybrid technology to the Contracting Authority's registered office (Cracow, 1 Złota Jesień Estate), in accordance with the description and requirements contained in Appendix 1A to the Contracting Authority. Specification.
• The validity period / warranty period of the subject of the order must not be shorter than 12 months from the date of delivery.
• At the request of the Contracting Authority, the Contractor undertakes to conduct training for the Contracting Authority's staff at its headquarters – Złota Jesień 1, Kraków along with the first delivery of the goods to the Hospital Pharmacy
within the deadline agreed in writing with the Ordering Party, but no later than within 30 days from the date of conclusion of the agreement.
• The Contractor will be obliged to submit the attendance list from the conducted training, with the signatures of the trained users confirming their actual presence, to the Hospital Pharmacy within 5 working days from the date of the training.
• The Contracting Authority reserves the right to address the Contractors at the stage of examination and evaluation of bids
to make available samples of the offered item of the order to test and check them. The costs associated with this shall be borne by the Contractor.
• Bids that do not have the full scope of the subject matter of the contract will be rejected.
• Other terms and conditions of the contract are specified in the draft agreement constituting Appendix 3A to the Terms of Reference.
2. rental of hybrid generators – 2 pcs. (apparatus), as described and requirements in Annex I to
No. 1B to the SWZ.
• The apparatus limits are described in Appendix 1B to this SWZ
• Additional conditions concerning the subject matter of the contract:
a. The Contractor shall provide the Contracting Authority with a warranty for the duration of the rental of the apparatus, for a period of min. 24 months from the date of signing the agreement;
b. The contractor will bear all costs related to the maintenance of the leased object (e.g. technical inspections in accordance with the manufacturer's recommendations, ongoing repairs and maintenance) during the term of the contract.
c. the costs of periodic inspections, maintenance of the apparatus, including consumables, shall be covered by the Contractor,
d. if it is necessary to replace wear parts during the technical inspection, the cost of these parts is borne by the Contractor - after each inspection it ensures the issuance of a certificate of efficiency of the apparatus,
e. The Contractor will provide training to the employees of the Medical Equipment Department, General Surgery and Oncology Department in the number and time to ensure proper and safe operation of the devices within 2 working days from the date of delivery of the device. The cost of disposable materials used during the training will be covered by the Contractor.
