The subject of the order is: "supply of 4 Type C sanitary ambulances for the needs of emergency medical services", described in detail in the ToR and the description of the subject of the contract in Annex 2 to the ToR.

The subject of the contract is financed from a special-purpose subsidy provided by the Minister of Health for the purchase of ambulance for the needs of emergency medical services, which must be used by 31/12/2019.

The subject of the order is: "supply of 4 Type C sanitary ambulances for the purpose of emergency medical services", described in detail in the description of the subject of the contract contained in Annex 2 to the ToR.

The subject of the contract is financed from a special-purpose grant provided by the Minister of Health for the purchase of an ambulance, for the purpose of emergency medical services, which must be used by 31/12/2019.

The offered object of the contract must have documents authorizing it to road traffic in the territory of the Republic of Poland, enabling its registration and operation immediately after receipt by the Employer in accordance with applicable law, i.e.

1) meet the requirements of the current PN-EN 1789 + A2 and PN-EN 1865-1 + A1 standards to the extent relevant to this proceeding;

2) have a valid approval (for a car with a body) issued in accordance with the provisions of Chapter III, Chapter 1a of the Act of 20.6.1997 Road Traffic Law (consolidated text Journal of Laws of 2018, item 1990, as amended) ;

3) meet the conditions set out in the provisions regarding the technical conditions of vehicles and the scope of their necessary equipment in accordance with the Regulation of the Minister of Infrastructure of 31.12.2002 (consolidated text Journal of Laws of 2016, item 2022);

4) meet the conditions set out in the provisions regarding the designation of the State Emergency Medical System in accordance with the Regulation of the Minister of Health of October 18, 2010 (consolidated text Journal of Laws of 2018, item 1251);

5) meet the conditions set out in the provisions of the Regulation of the Minister of Infrastructure of 27.9.2003 on detailed activities of the authorities in matters related to the entry into service of vehicles and model documents in these matters (consolidated text of the Journal of Laws of 2016, item 1088 as amended).

The subject of the order must be manufactured in 2019, brand new, unused, not exhibition, not demonstrative, complete, instruction in Polish, must be ready for use and have the equipment and parameters indicated in the description of the subject of the order, Annex 2 to this ToR.

Medical equipment included in the development must have documents admitting to trading and use in accordance with the Act of 10.5.2010 on medical devices (consolidated text Journal of Laws of 2019, item 175).

The Contractor shall provide a guarantee for the delivered ambulances, whose validity will start from the date of signing a positive ambulance collection report:

1) for the base vehicle - for a period of not less than 24 months;

2) for medical buildings - for a period of not less than 24 months;

3) for a varnish coat - for a period of not less than 24 months;

4) warranty on body perforation - for a period of not less than 60 months;

5) for medical equipment - for a period of not less than 24 months.

At the risk and cost of the contractor, the contractor should deliver the subject of the contract to the seat of the Employer.
