1.The subject of the contract is the successive delivery of laboratory reagents along with the lease of analyzers for the Independent Public Health Care Institution – Dr. J. Dietl Hospital in Krynica-Zdrój within 36 months (in the case of packages No. 1,2,3,4,5,6,8,9) and 12 months from the date of signing the contract in the case of package No. 7.
2. The subject of the order consists of 9 packages. A detailed assortment and quantity list can be found in the "Assortment and price form" - Appendix No. 2 to the SWZ.
