The subject of the contract is the round-the-clock and all days of the year provision of sanitary transport services in the country, mostly in the Lublin Voivodeship and the following poviats: Zamość, Biłgoraj, Hrubieszów and Tomaszów
The scope of round-the-clock service includes the carriage of:
patients requiring consultation of a specialist doctor or a diagnostic test in another medical entity
patients to another medical entity in order to continue treatment in the case of diseases threatening health or life, when treatment cannot be continued in the Ordering Party's departments;
people with locomotor dysfunction, unable to use mass transport to the place of residence after treatment
test materials
The number of kilometers expected to be completed - 275 000 km.

The scope of round-the-clock service includes the carriage of:
patients requiring consultation of a specialist doctor or a diagnostic test in another medical entity
patients to another medical entity in order to continue treatment in the case of diseases threatening health or life, when treatment cannot be continued in the Ordering Party's departments;
people with locomotor dysfunction, unable to use mass transport to the place of residence after treatment

Under the agreement, transport services will be provided for night and holiday health care on weekdays from 18.00 to 8.00 and non-working days – around the clock.
The number of kilometers expected to be completed - 275 000 km.
Transport performed by ambulances with the crew:
a) driver + paramedic – 52 000 km
b) driver + paramedic – 165 000 km
c) driver – 58 000 km
(drivers with first aid licences)
The number of kilometers depends on the number of patients requiring transport by medical transport, therefore the total number of kilometers will not exceed 275,000 km during the term of the contract. The Parties agree that any reduction in the degree of implementation of the contract will cover no more than 20% in relation to the quantities specified in the SWZ.
Vehicles intended for the execution of the order should meet the requirements in accordance with the current PN-EN 1789 standard or equivalent.
The Contractor is obliged to provide the services being the subject of the contract with technically efficient vehicles and perform them with due diligence.
Vehicles intended for the execution of the subject of the contract should:
- have third party liability and accident insurance
- have up-to-date, allowing for traffic technical examinations
- have a medical opinion about the vehicle
- be kept in order,
The cast of the team will be determined each time in the transport order.
The maximum travel time is, in the case of orders submitted:
at least one day in advance – arrival at a specified hour ± 15 minutes,
on the day of departure – up to 60 minutes
The Contractor undertakes to:
- follow the recommendations of the Quality Committee and other Committees, Commissions and Teams operating at the Contracting Authority
- reliable keeping of documentation necessary for the performance of this contract, in accordance with generally applicable laws, as well as legal regulations issued by competent health care institutions, including the National Health Fund
- proceedings in accordance with the terms of contracts concluded by the Contracting Authority with institutions financing health services,
- the person accepting the Order undertakes to submit to the control of the National Health Fund in the scope resulting from this contract on the terms set out in the Act of 27 August 2004 on health care services financed from public funds (Journal of Laws of 2021, item 1285, as amended)
The manner of implementation of the subject of the contract is specified in Annex 4 – Draft provisions of the contract – an integral part of the SWZ.
