Description:

This service will provide pre-bookable appointments between the hours of 18:30 and 20:00 (as a minimum) Monday to Friday, and during the day on Saturdays, Sundays and Bank Holidays (including Christmas and Easter) from 1.4.2019 for patients registered with a Southend CCG GP practice, or resident within the borough, Clinical staff must include a GP who can be accompanied by another clinician such as ANPs, Practice nurses, ECPs, Pharmacists, Mental health link workers, Healthcare assistants etc. A receptionist will also be required throughout at both sites. The service must deliver a total of 74 hours per week (excluding weeks with a bank holiday). This should be split across 2 sites, details of which may be found in the tender documentation.



The CCG needs to commission an additional 95 hours of clinical time per week for the borough in order to meet NHS England Enhanced Access target. A portion of this time will be provided during core hours and therefore not part of this contract. This contract only relates to additional provision during weekday evenings, Saturdays, Sundays and Bank Holidays. The service aims to:

— Ensure a greater availability of same day GP / PN appointments for patients with urgent clinical need within practices and across the locality, supporting wider system resilience,

— There should be effective cross-system working with:

— — EPUT Community Services including care co-ordination,

— — SUHFT – GP streaming and front door model

— Improve patient experience and perception of access, regardless of ethnicity, age, disability, sex, gender reassignment, religion/belief or sexual orientation, and to include access to appropriate translation services,

— Improve patient choice by offering a range of appropriate health professionals during service hours, ensuring a joined-up service between primary care, urgent care and social care where appropriate,

— Increase patient and carer understanding of self-limiting illness and steps to minimise the impact of illness,

— Improve staff satisfaction by introducing new ways of working,

— Reduce and prevent acute care unplanned emergency attendances and emergency admissions.
