1. The subject of the order is the delivery of an X-ray machine with a motorized C arm and 3D imaging, as well as an operating table with equipment for orthopedic procedures.

2. The equipment offered must meet the requirements of the Act of 20 May 2010 on medical devices. As part of the subject of the contract, the Contractor is obliged in particular:

• deliver complete equipment to the Employer's headquarters,

• assemble, configure, run and test the delivered equipment (in the case of part 1 perform acceptance tests),

• train the Employer's designated employees,

• issue and issue to the Ordering Party a warranty card for the subject of the order after prior agreement of the warranty conditions in accordance with the content of the offer and taking into account the requirements of the ToR,

• provide the Employer with the instruction manual in Polish, technical passport and technical documentation necessary for proper use,

• provide free warranty service for the equipment during the warranty period.

1. The subject of the order (part 1) is the delivery of an X-ray machine with a motorized C arm and 3D imaging for Dolnośląski Centrum Rehabilitacji Sp. z o. o. in Kamienna Góra.

2. The equipment offered must meet the requirements of the Act of 20 May 2010 on medical devices (i.e. Journal of Laws of 2020, item 186). As part of the subject of the contract, the Contractor is obliged in particular:

• provide complete equipment to the Employer's headquarters (Pavilion 1A - Operating Block),

• assemble, configure, run and test the delivered equipment (perform acceptance tests),

• train the Employer's designated employees (theoretical and practical training),

• issue and issue to the Ordering Party a warranty card for the subject of the order after prior agreement of the warranty conditions in accordance with the content of the offer and taking into account the requirements of the ToR,

• provide the Employer with the instruction manual in Polish, technical passport and technical documentation necessary for proper use,

• provide free warranty service for the equipment during the warranty period.

2. The equipment offered must meet the requirements of the Act of 20 May 2010 on medical devices (i.e. Journal of Laws of 2020, item 186). As part of the subject of the contract, the Contractor is obliged in particular:

• provide complete equipment to the Employer's headquarters (Pavilion 1A - Operating Block),

• assemble, configure, run and test the supplied equipment,

• train the Employer's designated employees (theoretical and practical training),

• issue and issue to the Ordering Party a warranty card for the subject of the order after prior agreement of the warranty conditions in accordance with the content of the offer and taking into account the requirements of the ToR,

• provide the Employer with the instruction manual in Polish, technical passport and technical documentation necessary for proper use,
