The subject of the contract is social services within the meaning of Article 7(3). The subject of the contract is the provision of personal assistant services for a disabled person (part I) and respite care services (part II).
Provision of personal assistant services for a disabled personThe subject of the contract is a task aimed at increasing the chances of people with disabilities to lead a more independent, independent and active life and striving to improve their functioning in the environment. The implementation of the task is co-financed by the Solidarity Fund. The order in this part should be performed in accordance with the rules set out in the Program of the Minister of Family and Social Policy "Personal assistant of a disabled person" – 2023 edition (hereinafter referred to as "Program No. 1"), which constitutes Annex 4 to the SWZ. Assistance services will be provided to disabled residents of Ruda Śląska, including children (hereinafter referred to as Program Participants), indicated to the Contractor by the Contracting Authority. Estimated number of people using the services: 51 people, including 6 children. Total number of planned hours of assistant services: 33,960 of which 2,160 are for children
The provision of respite care servicesThe subject of the contract is the implementation of a task aimed at relieving family members or carers of disabled people by supporting them in their daily duties or providing temporary replacement, thanks to which people involved in caring on a daily basis will have time that they will be able to spend on rest and regeneration, as well as on dealing with necessary matters. The implementation of the task is co-financed by the Solidarity Fund. The order in this part should be performed in accordance with the rules set out in the Program of the Minister of Family and Social Policy "Respite Care" – 2023 edition (hereinafter referred to as "Program No. 2"), which constitutes Annex 5 to the SWZ. Respite care services will be granted to residents of Ruda Śląska, family members or guardians directly caring for disabled people, including children, living in a common household (hereinafter referred to as Program Participants), and provided to their charges, indicated to the Contractor by the Contracting Authority. Estimated number of disabled people using the services: 39 people, including 11 children. Total number of planned hours of respite services: 9,360 of which 2,640 are for children.
