IV. Description of the Order

1. The subject of the order is the successive delivery of disposable equipment to the NZOZ Hospital Pharmacy of the County Hospital in Dzierżoniów Sp z o.o. with its registered office in Dzierżoniów

1 Cichej Street, 58-200 Dzierżoniów.

2. The subject matter of the contract is described in detail in Annex 1A to the SWZ - assortment-price forms and divided into parts, in which the forecast consumption for a period of 12 months is presented.

3. Number of parts: 58 Parts with the possibility of submitting partial bids for each part separately.
