The subject of the order is the delivery and assembly together with the design and commissioning of 37 solar collector installations and 50 photovoltaic micro installations under the program "Utilization of solar energy in the Nowe Ostrowy Commune" on private real estate and public buildings located in 21 towns of the Nowe Ostrowy Commune, divided into 2 parts.

PART I: delivery and assembly together with the design and commissioning of 37 solar collector installations: preparation of design documentation 37 solar collector installations on private buildings or on land belonging to the commune residents, as well as delivery of installation components and assembly of solar collector installations on the spot based on completed design documentation along with commissioning and instruction. Three types of solar collector installations are envisaged, depending on their power and volume of capacitive heaters (sets), in quantities:
• Set I: 2 flat solar collectors with a capacity of min. 2690 W, feeding the DHW cylinder with a net volume of min. 240 dm3 in quantity of 21 items
• Set Ia: 2 flat solar collectors with a capacity of min. 2690 W, feeding the DHW cylinder with a net volume of min. 270 dm3 in quantity of 1 item
• Set II: 3 flat solar collectors with a capacity of min. 4035 W, feeding the DHW cylinder with a net volume of min. 270 dm3 in the amount of 15 items

8. List of statements or documents confirming the fulfillment of the conditions for participation in the procedure and no grounds for exclusion
To confirm the conditions of participation in the procedure and the lack of grounds for exclusion, the contractors shall submit:
8.1. Along with the offer, JEDZ's statement in electronic form with a qualified electronic signature in order to pre-confirm the conditions of participation in the procedure and the lack of grounds for exclusion. Statements of entities submitting the offer jointly and entities sharing the potential submitted on the JEDZ form should be in the form of an electronic document, signed by a qualified electronic signature by each of them to the extent that they confirm the circumstances referred to in art. 22 (1) of the PZP Act, i.e. meeting the conditions for participation in the procedure and no grounds for exclusion to the extent that each of the contractors shows that they meet the conditions for participation in the procedure and no grounds for exclusion (Contractors must jointly demonstrate compliance with the conditions for participation in the procedure, but no grounds each is excluded separately).
8.1.1. The Purchaser allows in particular the following format of sent data: .pdf, .doc, .docx, .xps, .odt, rtf.
8.1.2. The contractor fills in EAT, creating an electronic document. Can use the ESPD tool or other available tools or software that allow you to fill in the EAT and create an electronic document, in particular in one of the abovementioned formats.
8.1.3. After the contractor has created or generated the EAT electronic document, the contractor signs the abovementioned a document with a qualified electronic signature issued by a qualified trust service provider that is an entity providing certification services - an electronic signature that meets the security requirements set out in the Act.
8.1.4. The contractor is obliged to complete the EAT form in the scope of: Part II, sections: A, B, C, D; Part III sections: A, B, C, D; Part IV section: α; Part VI - signature.

8.2. Within 3 days of posting on the website the information referred to in art. 86 section 5 of the Public Procurement Law, the Contractor shall provide the Employer, without additional request, with a statement
On belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law in order to demonstrate the lack of grounds for exclusion from the public procurement procedure in the circumstances referred to in art. 24 paragraph 1 point 23) of the PZP Act, .....

PART II: delivery and assembly along with the design and commissioning of 50 photovoltaic micro installations: preparation of design documentation of 50 photovoltaic micro installations on private buildings or land belonging to the commune residents and on public buildings, delivery of installation elements and carrying out of the installation assembly together with accompanying infrastructure and connection to the internal power installation based on the completed project documentation, including commissioning and instruction. The photovoltaic micro installation must consist of the following elements: photovoltaic panels mounted on the support structure, DC / AC inverter with the function of measuring the electricity produced, installation of direct and alternating current, measuring and billing system at the place of delivery / reception of electricity. The following types of solar micro-installations are expected, depending on their power, in quantities:
On private properties (43 items in total):
• Set of solar panels with a total power of min. 2.04 kW, 24 pieces;
• Set of solar panels with a total power of min. 3.06 kW, 13 items;
• Set of solar panels with a total power of min. 4.08 kW, 5 items;
• Set of solar panels with a total power of min. 5,10 kW, 1 item.
On public buildings (total 7):
• Set of solar panels with a total power of min. 5,10 kW, 1 item;
• Set of solar panels with a total power of min. 9.86 kW, 1 item;
• Set of solar panels with a total power of min. 14.96 kW, 1 item;
• Set of solar panels with a total power of min. 24.82 kW, 2 items;
• Set of solar panels with a total power of min. 27.88 kW, 1 item;
• Set of solar panels with a total power of min. 39.78 kW, in quantity of 1 piece.

8.2. Cd .... according to template 4 to IDW. Along with submitting a declaration of belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law, the contractor should provide, under pain of exclusion from the contract award procedure, evidence that links with another contractor do not lead to distortion of competition in the contract award procedure. The statement should be submitted in the original in the form of an electronic document bearing a qualified electronic signature or an electronic copy of the document confirmed as being true to the original.
8.3. Before awarding the contract, the contracting authority will call the contractor whose tender has been rated the highest to submit it within a specified time limit, not shorter than 10 days, valid as at the date of submitting the following statements or documents:
8.3.1. To confirm compliance with the condition referred to in point 5.2.2. SIWZ information from the bank or cooperative savings and credit union confirming the amount of financial resources or the contractor's creditworthiness, issued not earlier than 1 month before the deadline for submitting bids; the document should be provided in the original in the form of an electronic document or an electronic copy of the document certified as being true to the original.
8.3.2. To confirm compliance with the condition referred to in point 5.2.3.1. ToR - list of deliveries / services / construction works carried out not earlier than within the last 5 years before the deadline for submission of tenders, and if the period of business activity is shorter - in this period, together with their value, subject, date of completion and entities,. ....
