Delivery to Zakopane District Hospital of disposable and multi-use medical equipment

1. The subject of the tender is the selection of a contractor who will perform supplies to the contracting authority consisting in the successive delivery by the contractor to the Dr. Tytus Chałubiński District Hospital of disposable and reusable medical equipment divided into 80 (eighty) Packages, the division and detailed scope of which are presented in points 4.12 – 4.91 of the SWZ in question.

2. The Ordering Party reserves that the delivered subject of the order must have the required by current legal regulations, e.g. permits, certificates, certificates, attestations and markings provided for in the Act on medical devices proving the required admission to use and circulation in Poland in accordance with the Regulation of the Minister of Health on essential requirements and conformity assessment procedures for medical devices.

3. The subject of the order must be marked by the manufacturer in such a way as to identify both the product and the manufacturer.

Package - No. 1

1. The object of this part of the contract is the choice of the contractor who will provide to the contractor deliveries consisting in the successive delivery by the contractor to Dr. Titus Chałubiński District Hospital in Zakopane, Kamieniec 10 street, 34-500 Zakopane, medical equipment single and multiphase use included: Nitrile gloves — according to the minimum technical, quality, operational and functional parameters specified in section 4.12 of the SWZ in question — 3 assortment items.

2. The Contracting Authority stipulates that the subject of the contract offered, bearing the expiry date, must have an appropriately long validity period allowing free use for a minimum period of 12 months from the date of delivery.

3. The purpose of this proceeding is to obtain a product with technical, quality, operational and functional parameters specified in the SWZ, rather than to purchase disposable and multi-use medical equipment of a particular brand or manufacturer. For these reasons, the Contracting Party took due care to ensure that the subject matter of the contract is not described by an indication of trademarks, patents or origin, source or specific process which could lead to the privileging or elimination of certain contractors or products. If, in spite of this, it is found that there are such indications in any place of the SWZ and in the Annexes thereto, they should not be regarded as requirements relating to the subject of the contract, and should be considered only in terms of indications of an informational nature (non-binding contractors). For these reasons, an offer which will not conform to such indications will not be considered incompatible with the SWZ content and will not be rejected for these reasons.

4. The Contracting Authority stipulates that delivery for the duration of the contract will be carried out successively on the basis of partial orders placed by the Head of the Supply Section of the Hospital in Zakopane by fax or e-mail.

5. The Contracting Authority stipulates that partial deliveries will take place as long as 5 calendar days from the placing of the partial order from 8:00 to 14:00 in properly labelled packages. Each delivery batch must contain: name of the range, period of validity (if applicable), series number, name and address of the manufacturer/supplier. In the event that the last day of delivery falls on the day off work, the delivery will be on the first business day after the set date.

6. The Contracting Authority informs that the quantity of individual single-use and multi-use medical equipment covered by this tender procedure is an estimated size and may change during the implementation of the contract in question, reduction of no more than 20%, depending on the actual needs of the contracting authority.

7. Conditions for the execution of the order are contained in the draft contract, which is annex 6 of the SWZ.

1. The subject of this part of the contract is the selection of the contractor who will provide the contracting authority with deliveries consisting in the successive delivery by the contractor to the Dr. Tytus Chałubiński District Hospital in Zakopane, Kamieniec 10, 34-500 Zakopane, of disposable and reusable medical equipment covered by: Spatulas, blades, crimping machines, identification bands, foil aprons, shorts, enema sets, diapers, disposable surgical clothes, instruments, drains, brushes, casings, scissors, stases, syringes and thermometers, etc. – according to the list of minimum technical, quality, operational and functional parameters specified in point 4.21 of the SWZ – 45 assortment items.

2. The Contracting Authority stipulates that the subject of the contract offered, bearing the expiry date, must have an appropriately long validity period allowing free use for a minimum period of 12 months from the date of delivery.

3. The purpose of this proceeding is to obtain a product with technical, quality, operational and functional parameters specified in the SWZ, rather than to purchase disposable and multi-use medical equipment of a particular brand or manufacturer. 2. The Contracting Authority stipulates that the subject of the contract offered, bearing the expiry date, must have an appropriately long validity period allowing free use for a minimum period of 12 months from the date of delivery.

If, in spite of this, it is found that there are such indications in any place of the SWZ and in the Annexes thereto, they should not be regarded as requirements relating to the subject of the contract, and should be considered only in terms of indications of an informational nature (non-binding contractors). For these reasons, an offer which will not conform to such indications will not be considered incompatible with the SWZ content and will not be rejected for these reasons.

4. The Contracting Authority stipulates that delivery for the duration of the contract will be carried out successively on the basis of partial orders placed by the Head of the Supply Section of the Hospital in Zakopane by fax or e-mail.

5. The Contracting Authority stipulates that partial deliveries will take place as long as 5 calendar days from the placing of the partial order from 8:00 to 14:00 in properly labelled packages. Each delivery batch must contain: name of the range, period of validity (if applicable), series number, name and address of the manufacturer/supplier. In the event that the last day of delivery falls on the day off work, the delivery will be on the first business day after the set date.

6. The Contracting Authority informs that the quantity of individual single-use and multi-use medical equipment covered by this tender procedure is an estimated size and may change during the implementation of the contract in question, reduction of no more than 20%, depending on the actual needs of the contracting authority.

7. Conditions for the execution of the order are contained in the draft contract, which is annex 6 of the SWZ.

The Contracting Authority informs that this procedure will be conducted according to the so-called "Reverse Procedure", in accordance with the provisions of Article 139 of the Public Procurement Law. As part of this procedure, the contracting authority will first examine and evaluate the tenders, and then select the contractor whose tender has been rated the highest, in terms of the absence of grounds for exclusion and meeting the conditions for participation in the procedure.

The object of this part of the contract is the choice of the contractor who will provide to the contractor deliveries consisting in the successive delivery by the contractor to Dr. Titus Chałubiński District Hospital in Zakopane, Kamieniec 10 street, 34-500 Zakopane, medical equipment Disposable and multiphase use covered by: videoprintere paper and ultrasound and ECG gel — according to the summary of the minimum technical, quality, operational and functional parameters specified in section 4.20 of the SWZ in question - 5 assortment items.

2. The Contracting Authority stipulates that the subject of the contract offered, bearing the expiry date, must have an appropriately long validity period allowing free use for a minimum period of 12 months from the date of delivery.

3. The purpose of this proceeding is to obtain a product with technical, quality, operational and functional parameters specified in the SWZ, rather than to purchase disposable and multi-use medical equipment of a particular brand or manufacturer. For these reasons, the Contracting Party took due care to ensure that the subject matter of the contract is not described by an indication of trademarks, patents or origin, source or specific process which could lead to the privileging or elimination of certain contractors or products.
