1) The subject of the contract is the supply of surgical and diagnostic gloves for the needs of the John Paul II Provincial Hospital in Bełchatów. A detailed description of the subject matter of the contract can be found in Appendix 2 to the Terms of Reference.

2) The Contracting Authority has divided the subject matter of the contract into 10 parts (packages from 1 to 10), as described in Chapter XXIII of the Terms of Reference.

3) The subject of the contract offered in the tender has all the necessary certificates, including declarations of conformity, complies with the standards in force in the European Union, meets the requirements set out in the Act of 7 April 2022 on medical devices and the marketing and use authorisation required by the overriding regulations.

4) Other requirements concerning the performance of the contract and the principles of cooperation between the contracting authority and the contractor are specified in the proposed provisions of the public procurement agreement, constituting Appendix No. 3 to the Terms of Reference.
