1. The subject of the contract is the supply of personal protective equipment under the project "Development and maintenance of rescEU CBRN stockpiles in Poland" – stage 1.
2. The order has been divided into ten Lots.
3. A detailed description of the subject matter of the contract, in particular the specification of the technical and functional parameters of the subject matter of the contract, with a detailed division into Parts, is attached as Appendix No. 1 to the Terms of Reference – "Description of the subject matter of the contract".
4. The terms and conditions of performance of the subject matter of the contract, individually for each of the Parts of the Contract, are specified in Appendix No. 2A – 2J to the Terms of Reference – "Draft Contractual Provisions".
1. As part of Part 1 of the contract, the Contractor will be obliged to deliver air purification devices with a full face mask, in the amount of 500 sets (sets), in accordance with the technical specification specified in Appendix 1 to the Terms of Reference – "Description of the subject of the contract".
2. Regardless of the service consisting in the delivery of the equipment referred to in paragraph 1 above, the Contractor shall also be obliged to:
A. conduct a one-day training in the field of operation, maintenance and use of the delivered equipment for not less than 6 and not more than 9 employees of the Ordering Party or other persons designated by the Ordering Party,
B. permanent and visible marking of each package of each of the devices with the logo of the European Union flag together with a statement on the method of financing: "Funded by the European Union", the template of which is Appendix No. 1A to the Terms of Use – "Model logotype", subject to:
a. dimensions of the EU flag – the dimensions of the EU flag may not be smaller than:
10.5 cm (height) x 16 cm (width), with an appropriately visible statement on the method of financing,
1 cm (height), with a suitably visible statement on the method of financing – for packaging with dimensions less than or equal to 21 cm x 29.7 cm,
b. the necessity to maintain the proportions of the logotype and the statement on the method of financing in relation to each other,
c. the necessity to maintain the proportions of the logotype and the statement on the method of financing in relation to the packaging – clear visibility of the marking,
d. the necessity to maintain the colour scheme of the logotype and the statement on the method of financing (compliance with the template),
e. The Ordering Party allows the packaging of the device to be marked with the use of stickers, stickers and similar materials, if they ensure a permanent bond with the packaging of the device,
f. The Ordering Party allows labelling of the collective packaging of devices only if the equipment in question is packaged by the manufacturer, i.e. when the device packed in unit packaging is usually transported in a collective packaging. In particular, the Contracting Authority does not allow the combination of unit packages into collective packages or collective packages customarily packaged by the manufacturer, into larger collective packages (packages "packaged" by the Contractor) in order to avoid the obligation of the Contractor to mark each unit and/or collective packaging of the device,
C. providing the Contracting Authority with a quality guarantee for the delivered equipment at least until 31 December 2026, under which the Contractor will be obliged to:
a. removal of defects and defects of the device,
b. carry out or ensure periodic inspections of the device or its individual components, as required and/or recommended by the manufacturer of the device, including to ensure ongoing (during the period of operation) and/or periodic (in particular during or as a result of periodic inspection) replacement and/or replenishment of consumables and/or replacement parts,
at the sole expense of the Contractor and under the conditions set out in Appendix 2A to the Terms of Use – "Draft Contractual Provisions".
3. A detailed description of the subject matter of the contract, in particular the specification of the technical and functional parameters of the subject matter of the contract, is attached as Appendix No. 1 to the Terms of Reference – "Description of the subject matter of the contract".
4. The terms and conditions of the performance of the subject matter of the contract are set out in Appendix No. 2A to the Terms of Reference – "Draft Contractual Provisions".
1. As part of Lot 10 of the contract, the Contractor will be required to provide:
a. chemical protection gloves in the amount of 10,000 pairs,
b. 10,000 pairs of biological protection gloves,
c. 5,000 pairs of chemical/biological protection shoes,
in accordance with the technical specification specified in Appendix 1 to the Terms of Reference – "Description of the subject matter of the contract".
2. Regardless of the service consisting in the provision of personal protective equipment referred to in paragraph 1 above, the Contractor shall also be obliged to:
A. permanent and visible marking of each package of each of the personal protective equipment with the logo of the European Union flag together with a statement on the method of financing: "Funded by the European Union", the template of which is Appendix No. 1A to the Terms of Use – "Logo Template", subject to:
a. dimensions of the EU flag – the dimensions of the EU flag may not be smaller than:
10.5 cm (height) x 16 cm (width), with an appropriately visible statement on the method of financing,
1 cm (height), with a suitably visible statement on the method of financing – for packaging with dimensions less than or equal to 21 cm x 29.7 cm,
b. the necessity to maintain the proportions of the logotype and the statement on the method of financing in relation to each other,
c. the necessity to maintain the proportions of the logotype and the statement on the method of financing in relation to the packaging – clear visibility of the marking,
d. the necessity to maintain the colour scheme of the logotype and the statement on the method of financing (compliance with the template),
e. The Contracting Authority allows the packaging of personal protective equipment to be labelled with stickers, stickers and similar materials, if they ensure a permanent bond with the packaging of the personal protective equipment,
f. The Contracting Authority allows the labelling of the collective packaging of personal protective equipment only if the personal protective equipment is packaged by the manufacturer, i.e. when the personal protective equipment packed in the unit packaging is usually transported in the collective packaging. In particular, the Contracting Authority does not allow the combination of individual packages into collective packages or collective packages customarily packaged by the manufacturer into larger collective packages (packages "packaged" by the Contractor) in order to avoid the obligation of the Contractor to label each unit and/or collective packaging of personal protective equipment,
B. provide the Contracting Authority with a quality guarantee for the supplied personal protective equipment at least until 31 December 2026, under which the Contractor will be obliged to remove defects and defects in the personal protective equipment, at the sole expense of the Contractor and under the conditions set out in Appendix 2J to the Terms of Reference – "Draft Contractual Provisions".
3. A detailed description of the subject matter of the contract, in particular the specification of the technical and functional parameters of the subject matter of the contract, is attached as Appendix No. 1 to the Terms of Reference – "Description of the subject matter of the contract".

1. As part of Lot 2 of the contract, the Contractor will be obliged to deliver 3,000 full-face masks for RD 40 threaded filters and filter absorbers, in accordance with the technical specification specified in Appendix 1 to the Terms of Reference – "Description of the subject of the contract".
2. Regardless of the service consisting in the provision of personal protective equipment referred to in paragraph 1 above, the Contractor shall also be obliged to:
A. conduct a one-day training in the operation, maintenance and use of the supplied personal protective equipment for not less than 6 and not more than 9 employees of the Ordering Party or other persons designated by the Ordering Party,
B. permanent and visible marking of each package of each of the personal protective equipment with the logo of the European Union flag together with a statement on the method of financing: "Funded by the European Union", the template of which is Appendix No. 1A to the Terms of Use – "Logo Template", subject to:
a. dimensions of the EU flag – the dimensions of the EU flag may not be smaller than:

The Contracting Authority allows the labelling of the collective packaging of personal protective equipment only if the personal protective equipment is packaged by the manufacturer, i.e. when the personal protective equipment packed in the unit packaging is usually transported in the collective packaging. In particular, the Contracting Authority does not allow the combination of individual packages into collective packages or collective packages customarily packaged by the manufacturer into larger collective packages (packages "packaged" by the Contractor) in order to avoid the obligation of the Contractor to label each unit and/or collective packaging of personal protective equipment,
C. provide the Contracting Authority with a quality guarantee for the supplied personal protective equipment at least until 31 December 2026, under which the Contractor will be obliged to:
a. removal of defects and defects of personal protective equipment,
b. carrying out or ensuring periodic inspections of the personal protective equipment or its individual components, as required and/or recommended by the manufacturer, including to ensure ongoing (during the period of operation) and/or periodic (in particular during or as a result of periodic inspection) replacement and/or replenishment of consumables and/or replacement parts,
at the sole expense of the Contractor and under the conditions set out in Appendix 2B to the Terms of Reference – "Draft Contractual Provisions".

1. As part of Part 3 of the contract, the Contractor will be obliged to deliver 50 pieces of equipment for the transport of infectious patients, in accordance with the technical specification specified in Appendix 1 to the Terms of Reference – "Description of the subject of the contract".
2. Regardless of the service consisting in the delivery of the equipment referred to in paragraph 1 above, the Contractor shall also be obliged to:
A. conduct a one-day training in the field of operation, maintenance and use of the delivered equipment for not less than 6 and not more than 9 employees of the Ordering Party or other persons designated by the Ordering Party,

a. type 4 overalls, in the amount of 5,000 pieces,
b. type 5 overalls in the amount of 55,000 pieces,
in accordance with the technical specification specified in Appendix 1 to the Terms of Reference – "Description of the subject matter of the contract".

A. conduct a one-day training in the operation, maintenance and use of the supplied personal protective equipment for not less than 6 and not more than 9 employees of the Ordering Party or other persons designated by the Ordering Party,

A. conduct a one-day training in the operation, maintenance and use of the supplied personal protective equipment for not less than 6 and not more than 9 employees of the Ordering Party or other persons designated by the Ordering Party,

c. the necessity to maintain the proportions of the logotype and the statement on the method of financing in relation to the packaging – clear visibility of the marking,
d. the necessity to maintain the colour scheme of the logotype and the statement on the method of financing (compliance with the template),
e. The Contracting Authority allows the packaging of personal protective equipment to be labelled with stickers, stickers and similar materials, if they ensure a permanent bond with the packaging of the personal protective equipment,
f. The Contracting Authority allows the labelling of the collective packaging of personal protective equipment only if the personal protective equipment is packaged by the manufacturer, i.e. when the personal protective equipment packed in the unit packaging is usually transported in the collective packaging. In particular, the Contracting Authority does not allow the combination of individual packages into collective packages or collective packages customarily packaged by the manufacturer into larger collective packages (packages "packaged" by the Contractor) in order to avoid the obligation of the Contractor to label each unit and/or collective packaging of personal protective equipment,
C. provide the Contracting Authority with a quality guarantee for the supplied personal protective equipment at least until 31 December 2026, under which the Contractor will be obliged to:
a. removal of defects and defects of personal protective equipment,
b. carrying out or ensuring periodic inspections of personal protective equipment or its individual components, required and/or recommended by the manufacturer, including to ensure ongoing (during the period of operation) and/or periodic (in particular during or as a result of periodic inspection) replacement and/or replenishment of consumables and/or replacement parts,
at the sole expense of the Contractor and under the conditions set out in Appendix 2H to the Terms of Use – "Draft Contractual Provisions".
3. A detailed description of the subject matter of the contract, in particular the specification of the technical and functional parameters of the subject matter of the contract, is attached as Appendix No. 1 to the Terms of Reference – "Description of the subject matter of the contract".
4. The terms and conditions of performance of the subject matter of the contract are specified in Appendices No. 2H to the Terms of Reference – "Draft Contractual Provisions".
1. Under Lot 9 of the contract, the Contractor shall be required to provide:
a. RD 40 A2B2E2K2-P3 filter absorbers in the amount of 10,000 pieces,
b. RD 40 PF10-P3 filters in the amount of 10,000 pieces,
in accordance with the technical specification specified in Appendix 1 to the Terms of Reference – "Description of the subject matter of the contract".
2. Regardless of the service consisting in the provision of personal protective equipment referred to in paragraph 1 above, the Contractor shall also be obliged to:
A. conduct a one-day training in the operation, maintenance and use of the supplied personal protective equipment for not less than 6 and not more than 9 employees of the Ordering Party or other persons designated by the Ordering Party,
B. permanent and visible marking of each package of each of the personal protective equipment with the logo of the European Union flag together with a statement on the method of financing: "Funded by the European Union", the template of which is Appendix No. 1A to the Terms of Use – "Logo Template", subject to:
a. dimensions of the EU flag – the dimensions of the EU flag may not be smaller than:
10.5 cm (height) x 16 cm (width), with an appropriately visible statement on the method of financing,
1 cm (height), with a suitably visible statement on the method of financing – for packaging with dimensions less than or equal to 21 cm x 29.7 cm,
b. the necessity to maintain the proportions of the logotype and the statement on the method of financing in relation to each other,
c. the necessity to maintain the proportions of the logotype and the statement on the method of financing in relation to the packaging – clear visibility of the marking,
d. the necessity to maintain the colour scheme of the logotype and the statement on the method of financing (compliance with the template),
e. The Contracting Authority allows the packaging of personal protective equipment to be labelled with stickers, stickers and similar materials, if they ensure a permanent bond with the packaging of the personal protective equipment,
