The subject of the contract is the service of supporting the e-Health Centre in the development of ICT systems by providing the services of a consultant with a specialization – Project Manager. The contract includes the provision of a specialist whose required qualifications and experience are described in detail in Appendix 1 to the Terms of Reference – Description of the subject of the contract (OPZ). The expected total number of man-hours is a maximum of 5,016, including 3,008 rbh guaranteed and 2,008 rbh optional.

The expected total number of man-hours is a maximum of 5,016, including 3,008 rbh guaranteed and 2,008 rbh optional.

2. The contract has not been divided into lots because it concerns the provision of services by one person.

3. A detailed description of the item of the order is Annex 1 to SWZ - Description of the object of the order.

4. The Contractor is obliged to fulfil the order under the terms and conditions described in the model contract constituting Annex 2 to the SWZ.
