Equipping, free of charge and at their own expense, real estate notified by the Employer from the municipality and city of Nowe Brzesko with the necessary containers (containers) for collecting mixed waste, as well as the collection of containers after the end of the contract, with the necessary bags for the selective collection of municipal waste, in the bags necessary for collecting bottom ash. Collection, transport and management of mixed and segregated municipal waste. Collection, transport and management of bulky waste, electrical and electronic equipment, and used tires during traveling collections twice a year. 6. Collection, transport and management of bottom ash, construction and demolition waste from minor renovations constituting municipal waste. Organization of a mobile point for selective municipal waste collection along with service and collection at a place indicated by the Employer.

The subject of the contract includes the implementation of:

1. Equipping, free of charge and at own expense, real estate notified by the Employer from the commune and city of Nowe Brzesko with the necessary containers (containers) for collecting mixed waste, as well as collecting containers after the end of the contract.

2. Equipping, free of charge and at own expense, real estate notified by the Employer from the commune and city of Nowe Brzesko with the necessary bags for the selective collection of municipal waste.

3. Equipping, free of charge and at own expense, real estate notified by the Employer from the commune and the city of Nowe Brzesko with sacks necessary to collect furnace ash.

4. Collection, transport and management of mixed and segregated municipal waste.

5. Collection, transport and management of bulky waste, electrical and electronic equipment, and used tires during traveling collections twice a year.

6. Collection, transport and utilization of bottom ash.

7. Collection, transport and management of construction and demolition waste from minor renovations constituting municipal waste.

8. Organization of a mobile point for selective municipal waste collection along with service and collection at the place indicated by the Employer in the months of February, April, June, August, October, December. At the time of launching the municipal PSZOK, the contractor will be released from the mobile organization PSZOK, and as part of the contract he will be obliged to collect, transport and manage the waste supplied by the residents to PSZOK.

The detailed description and scope of the subject of the contract is set out in Annex 1 "Description of the subject of the contract".

Requirements referred to in art. 29 paragraph 3A of the Pzp.

1) Because the implementation of this contract must be carried out by carrying out activities in the conditions specified in Article. 22 § 1 of the Act of 26.6.1974 - labor code - for the contractor or subcontractor, at the place and time indicated by the contractor or subcontractor - the Employer, in accordance with art. 29 paragraph 3a of the PPL requires that the contractor or subcontractor employs persons performing activities falling under the so-called direct costs under an employment contract. this requirement applies to persons who perform activities directly related to the performance of works, in particular:

a) related to the receipt and export of municipal precipitation from property owners,

b) connected with operating the selective municipal waste collection point (PSZOK),

2) the method of documenting the employment of persons referred to in art. 29 paragraph 3a of the PPL Act.

The Contractor is obliged to submit to the Employer, within 7 days of the commencement of the implementation of the subject of the contract, a written statement of the contractor or subcontractor confirming that the employees of the contractor or subcontractor are employed on the basis of an employment contract within the meaning of the Act of 26.6.1974. - labor code including the minimum remuneration for work determined on the basis of art. 2 clause 3-5 of the Act of 10.10.2002 on the minimum remuneration for work (Journal of Laws of 2015, item 2008 and of 2016, item 1265). this statement should contain, in particular: the exact specification of the entity submitting the statement, the date of submitting the statement, an indication that the activities listed in point 1 are performed by persons employed under an employment contract together with an indication of the number of those persons, the type of employment contract and the number of full-time employment and the signature of the person entitled to make a statement on behalf of the contractor or subcontractor;

3) the Awarding Entity's powers to control compliance of the contractor with the requirements referred to in art. 29 paragraph 3a of the PPL Act. have been described in the draft contract as an attachment to the ToR;

4) sanctions for failure to meet the requirements referred to above have been specified in the draft contract which is an attachment to the ToR.
