The subject of the contract is the provision of health services in the field of occupational medicine for the Provincial Police Headquarters in Łódź, subordinate units and organizational units based in the city of Łódź
Pursuant to Article 257(1) of the Public Procurement Law, the Contracting Authority reserves the right to cancel the procurement procedure if the public funds that the contracting authority intended to allocate to finance all or part of the contract are not granted to it.
1. The Contractor shall provide medical services, including medical examinations and issuing medical certificates on this basis on the absence or existence of contraindications to work in the position occupied and consisting of:
a) periodic and control examination of police officers specified in the Regulation of the Minister of Internal Affairs and Administration of 9 January 2017 on periodic and control examinations of police officers (Journal of Laws of 2017, item 110, as amended);
b) preliminary, control and periodic examination of Police employees specified in Article 229 par. 1 and 2 of the Labor Code (Journal of Laws of 2022, item 1510);
(c) an additional medical examination in the cases specified in the Law of 26 June 1974. Labour Code (i.e. Journal of Laws of 2022, item 1510, as amended), carried out after the employee has reported the inability to perform the previous job, including medical examinations stating health contraindications to perform the previous work by a pregnant or nursing employee, referred to in Article 179 of the Act of 26 June 1974. Labour Code (i.e. Journal of Laws of 2022, item 1510, as amended), in accordance with the Regulation of the Minister of Health of 3 March 2006 on the method and procedure for issuing a medical certificate stating health contraindications to perform the previous work by a pregnant or breastfeeding employee (Journal of Laws of 2006, No. 42, item 292);
d) examination in order to obtain the right to drive a privileged vehicle in accordance with the Regulation of the Minister of Health of 5 December 2022 on medical examinations of persons applying for driving licenses and drivers (Journal of Laws of 2022, item 2503) and the Regulation of the Minister of Health of 8 July 2014 on psychological examinations of persons applying for driving licenses, drivers and persons performing work as a driver (i.e. Journal of Laws of 2022, item 165);
e) medical examination of helmsmen in accordance with the rules set out in the Regulation of the Minister of Health of 5 November 2003 on health conditions required of persons performing work on inland waterway vessels (Journal of Laws of 2003 No. 199, item 1949);
f) research and entry in sanitary and epidemiological books referred to in Articles 6 - 10 of the Act of 5 December 2008 on the prevention and control of infections and infectious diseases in humans (i.e. Journal of Laws of 2022, item 1657, as amended);
g) medical examination of divers and combat divers, in accordance with the Regulation of the Minister of Health of 17 September 2007 on health conditions for underwater work (Journal of Laws of 2022, item 2174) and the Regulation of the Minister of Internal Affairs and Administration of 25 May 2004 on the performance of underwater works in organizational units subordinate to or supervised by the minister competent for internal affairs (Journal of Laws of 2004, item 1468 No. 138);
h) protective vaccination of police officers and employees referred to in Article 20 of the Act of 5 December 2008 on the prevention and control of infections and infectious diseases in humans (Journal of Laws of 2022, item 1657, as amended), on the principles set out in the Regulation of the Minister of Health of 18 August 2011 on mandatory preventive vaccination
