The subject of the contract is the provision of cleaning services at the headquarters of the Healthcare Information Systems Center in the building at ul. Stanisława Dubois 5A in Warsaw.

2. The Contractor is obliged to perform the contract on the terms and conditions described in the contract template constituting Annex 2 to the ToR.
