Description:
Delivery of a Computer Tomograph for the Specialist Hospital in Chorzów

The subject of the contract is the purchase of a Computer Tomograph min. 64-row with equipment, delivery, installation, integration with the local PACS/HIS system, commissioning and training of staff and the necessary adaptation of the premises of the CT laboratory in the Specialist Hospital in Chorzów at 10 Zjednoczenia Street.

1. The subject of the order is the purchase of a Computer Tomograph min. 64-row with equipment, delivery, installation, integration with the local PACS/HIS system, commissioning and training of staff and the necessary adaptation of the premises of the CT laboratory in the Specialist Hospital in Chorzów at 10 Zjednoczenia Street.

2. Offered subject of the order and any additional elements included

it must consist of brand new, manufactured in 2022, non-reconditioned, non-exhibition, the apparatus after installation ready for use without the need to incur additional financial outlays.

3. The Ordering Party requires the integration of the supplied equipment with the one used

at the Ordering Party with the PACS/HIS system, enabling the fulfillment of functional provisions described in detail in Annex 1 to the offer form.

4. The Contractor is obliged to design the covers for the computer tomograph rooms, necessary for documentation for the Sanepid

5. The Contractor is obliged to perform acceptance tests

and specialized for the offered devices (tomograph and medical monitors in the descriptive station) in accordance with applicable regulations. Performing tests on the receipt of the camera and after significant warranty repairs throughout the warranty period.

6.The Contractor shall train the personnel operating the apparatus

at the registered office of the Ordering Party, details are presented in Annex 1 to the offer form.

7.The technical parameters are specified in detail in Annex 1 to the offer form entitled: "Technical parameters – Description – Parameters to be evaluated", which must be completed in full, the lack of entry will result in the rejection of the offer. Selected technical parameters for evaluation are marked with an asterisk ( * ). The maximum possible sum of technical points of the selected parameters is 100.

8.The subject of the order must meet the essential requirements set out in the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, item 974).

9. The subject of the contract included in the offer should have approvals

for use in health care facilities and meet the essential requirements set out in the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, item 974).

10.Depending on the type of equipment offered, the Ordering Party requires the delivery of documents specified in the Act of 7 April 2022 on medical devices, the Act of 6 September 2001 - Pharmaceutical Law (consolidated text Journal of Laws of 2020, item 944, as amended).

11.In the event that the subject of the contract described in the specification has not been classified as a medical device and in accordance with European directives and the Act on Medical Devices is not covered by declarations of conformity and is not subject to any entry in the register, the Ordering Party only requires the submission of an appropriate statement.

12. The Ordering Party allows the possibility of submitting equivalent offers.

13. The Contractor who refers to solutions equivalent to those described by the Ordering Party is obliged to demonstrate that the offered subject of the contract meets the requirements specified by the Ordering Party.

14. The Ordering Party informs that all indications of trademarks, patents or origin appearing in the documentation should be understood as defining the technical parameters or quality standards required by the Ordering Party. This means that in accordance with Article 99(5) and (6) of the Public Procurement Law (Journal of Laws of 2021, item 1129), these indications are accompanied by the words "or equivalent". The Ordering Party allows the possibility of presenting an equivalent assortment in the offer (other than the one given "by name" by the Ordering Party in the description of the subject of the contract) provided that the offered devices and materials will be of the same or better technical, quality, functional and functional parameters in relation to the devices and materials specified by the Ordering Party in the description of the subject of the contract.
