Execution of the project documentation — construction and implementation project, together with the decision to permit the implementation of the road investment for the task of the following: “Expansion of the Provincial Road No. 483 on the Szczerce — Brzezie section”:

(a) basic order — execution of project documentation — construction and implementation project, together with the decision to authorize the implementation of road investment for the task of “Expansion of provincial road No 483 on the Szczerce — Brzezie section”;

(b) the option — the supervision of the implementation of the task of the following: “Expansion of Provincial Road No. 483 on the Szczerców—Brzezie section”.

The Contracting Authority provides for the award, within 3 years from the date of the award of the basic contract, to the existing Contractor of services or works, of a contract consisting in the repetition of similar services or works in accordance with the subject of the basic contract, to the amount of 61 350,00 Net PLN.

The scope of the project documentation includes the expansion of the provincial road number 483 on the Szczerców—Brzezie section, i.e. from about km 33+486,00 to about km 34+485.

As part of the contract, multi-industry design documentation should be carried out allowing the decision to authorise the implementation of the road investment and the execution of all construction works necessary for the proper implementation of the investment.

The order includes the execution of the following elements:

— map for design purposes on paper and in numerical form,

- Land development plan,

— architectural and construction design of the road industry,

— the executive project of the road industry,

— architectural and construction project of drainage construction,

— executive project of drainage construction,

— architectural and construction project of the construction of the technological canal,

— executive project of the construction of the technology channel,

— architectural and construction project of the reconstruction of the teletechnical network,

— the implementation project of the reconstruction of the teletechnical network,

— architectural and construction project of the reconstruction of the power grid,

— the executive project of the reconstruction of the power grid,

— architectural and construction project of the construction of road lighting,

— the implementation project of the construction of road lighting,

— architectural and construction project of the reconstruction of the water supply network,

— the executive project of the reconstruction of the water supply network,

— architectural and construction project of the reconstruction of sanitary sewerage system,

— the executive project of the reconstruction of sanitary sewage system,

— technical design separately for each of the industries*,

— establishing the geotechnical conditions of the foundation,

— the project of organizing the movement for the duration of works,

— the project of a permanent movement organization,

— safety and health information,

— materials for obtaining waterproofing (including water operation with land registry discharge), together with obtaining a water-legal decision; water-legal consent,

— materials for obtaining an opinion on the application for a decision to authorise the implementation of road investment,

— materials for the application for a decision to authorise the implementation of road investment,

— the plan for cutting and planting greens,

— obtaining all other required arrangements and decisions, provisions in accordance with specific requirements, including the relevant Coordination Protocol,

— TSPACW — for each of the industries based on the TSPACW of the ZDW,

— a detailed object of works for each industry,

— an offer valuation form compliant with the TSPACW,

— investor cost estimate.

In the framework of the development of the design documentation, a technical project referred to in Article 34 (3) of the Construction Law should be drawn up to the extent that it will be required due to the peculiarities of the construction facilities falling within the scope of the investment.

Pursuant to Article 95 (1) of the Pzp Act, the Contracting Authority requires that the Contractor/Subcontractor employ persons pursuant to an employment contract during the execution of the contract, if the performance of these activities consists in the performance of the work in the manner laid down in Article 22 (1) of the Act of 26 June 1974 — Labor Code, i.e. persons performing the activities during the execution of the contract referred to in the tender valuation form, with the exception of persons performing independent technical functions in construction and persons carrying out activities related to the performance of surveying work, work design, including environmental studies.
