The subject-matter of the contract is the provision of care services and specialist nursing care services at the place of residence The present proceedings are divided into Parts I and II. The description of the subject of the contract is: a) "Model contract" (Annex 6 to the SWZ); b) "Detailed description of the subject of the contract" (Annex 13 to the SWZ)

I. The subject of the contract is the provision of basic care services in the period from 01.10.2023. to 30.09.2026r. -with the proviso that the earlier exhaustion of the maximum number of hours referred to in point 6 results in the expiration of the contract (at the place of residence (or temporary stay in the hospital at the address of a medical facility in the city) of persons who, due to age, illness or other reasons, require assistance, hereinafter referred to as service recipients in the following form:

- purchases of groceries and other household items

in the nearest store, keeping a book of expenses and settling accounts with the mentee from the money spent

- preparation and serving of meals

- delivery of a meal from catering establishments indicated by the recipient or by OPS

- help with eating a meal

- keeping clean tableware, kitchen and other household appliances for everyday use, serving the ward

- keeping sanitary equipment and sanitary facilities clean

- change of personal underwear, bed linen, bed transfer.

- washing the recipient's personal underwear, bed linen and clothing using a washing machine

- assistance in maintaining the personal hygiene of the recipient, assistance with changing diapers, underwear and clothing (except for people lying down)

- keeping clean the premises used by the recipient

- dealing with official matters, assistance in accessing various institutions, submitting applications for benefits paid by the tut. Center, payment of housing fees, submitting an application for a disability certificate, applications to ZUS, UM, organization of means of transport for charges, reporting for repair of home appliances and installations and other activities resulting from the justified individual needs of the recipient

- accompanying during walks

- burning in the furnace and/or / or bringing coal, ash removal

- window cleaning – applies only to single people with low income (1 time per year)

- garbage removal

- assistance in reaching institutions, e.g. WYDS, DDPS, WTZ and others

- ensuring contacts with the environment, maintaining the interests of the recipient

- assistance in obtaining documentation allowing to apply for benefits and cooperation with a social worker in this area

- ordering medical appointments, fulfilling prescriptions, supervising the timeliness and validity of medicines, reminding about pharmacotherapy, contact with the attending physician with the client's consent, reporting irregularities in the health condition to the family, family doctor, and in the case of single people to a social worker

- accompaniment during medical visits in the case of single people

- support in the care of pets in the case of single people

The subject of the contract is the provision of specialist nursing care services in the place of residence of persons who, due to age, illness or other reasons, require assistance, hereinafter referred to as beneficiaries in the following form:

- maintaining personal hygiene (bathing the patient in the bathroom or washing the patient in bed, washing and drying hair, washing the whole body, washing, shaving, cutting nails on hands and feet, cleaning dentures)

- making the bed and changing bed linen for people lying down, not getting out of bed alone

- laying the patient in bed

- assistance in physiological activities of people lying down and / or only sitting alone - change of diapers, foundation

- combing and dressing

- reminder about the need to take medication, reminder about the time of insulin administration, lubrication, patting, rubbing to reduce the risk of skin irritation and bedsores, body temperature measurement, measurement of blood pressure with the recipient's apparatus, sugar level test. Observing possible side effects of taking medications, reporting irregularities in health, taking medications, etc.

- preparation and administration of a meal to a person lying down, choking on medical indications for serving meals and drinks

- assisting the family in transferring the patient

- ordering medical appointments, and cooperation of the person providing services with a doctor and a health visitor, cooperation of a social worker with a person providing services and medical facilities (POZ, long-term nursing care team), observation of the patient and his state of consciousness, prescriptions. Contact with the attending physician with the consent of the recipient, accompaniment during medical visits

- transferring the patient to/from a wheelchair
