The subject of the contract are transport services, consisting in transporting 36 disabled pupils to kindergartens, schools and educational centres and their transport after classes to their places of residence with care.

The subject of the contract has been divided into 11 parts (routes).

The contracting authority allows partial bids to be submitted and informs that one contractor may submit bids for all lots, i.e. 11.

Descriptions of individual routes cannot be taken as definitively defined. The lengths of the individual routes and the beginning and completion times may not be final, they have been given to give contractors a common basis to draw up bids.

Detailed description of the subject matter of the order divided into parts is contained in the Description of the subject of the contract, which is Annex 7 to the SWZ.

1. Transport from the designated place to the Special Primary School No. 7 in Katowice, 5 Szkolna Street (SPS-7) and the School and Preschool Complex No. 3 in Katowice, 4 Grzegorzka Street and return.

Type of disability: SPS-7 - students and intellectual disability in a mild, moderate degree, with multiple disabilities - autism and moderate intellectual disability, ZSP-3 - integration classes for visually impaired, hearing-impaired, mildly and motor intellectual disabilities including aphasia, multiple disability, autism

Delivery at 7:45 to SPS-7 and 8:00 to ZSP-3, drop-off at 15:20 from ZSP-3 and 15:30 from SPS-7

ROUTE 1 – 3 children (route about 20 km/day)

A vehicle with a number of seats for a minimum of 5 children + 2 seats for the driver and guardian is required.

Transport from the designated place to the Municipal Kindergarten No. 50 with Integration Classes in Katowice, Mościckiego 2 (MP-50) and Municipal Kindergarten No. 67 with Integration Classes in Katowice, Al. Roździeńskiego 88 (MP-67) and return.

Type of disability: MP-50 - integration wards for children with intellectual and motor disabilities and a special ward for children with autism and couplings, MP-67 - integration classes for children with mild, moderate and severe intellectual disability, disability, motor disability, including aphasia, with autism and couplings, visually impaired.

Drop-off at 7:45 to MP-50, 8:00 MP-67, drop-off between 15:00 and 16:00.

ROUTE 10 – 2 children (route about 28 km / day)

A vehicle with a number of seats for a minimum of 4 children + 2 seats for the driver and guardian is required.

Transport from the designated place to the Municipal Kindergarten No. 35 in Katowice, 1 Rybnicka Street (MP-35) and Municipal Kindergarten No. 66 with M. Jeżowska Integration Classes in Katowice, 50 Brynicy Street (MP-66) and return.

Type of disability: MP-66 - integration wards for children with intellectual and motor disabilities, autism, multiple disabilities, MP-35 - motor disability, including aphasia.

Drop-off at 7:40 to MP-35, 8:00 MP-66, drop-off between 15:00 and 16:00.

ROUTE 11 – 3 children (route about 34 km / day)

A vehicle with a number of seats for a minimum of 5 children + 2 seats for the driver and guardian is required.

Transport from the place of residence to Primary School No. 58 with Maria Dąbrowska Integration Classes in Katowice, Al. B. Krzywoustego 11 (SP-58) and Janusz Korczak Primary School No. 66 in Katowice, B. Krzywoustego 7 (SP-66) and return.

Type of disability: SP-58 - students with mild and moderate intellectual disability, motor disability including aphasia, multiple disability, hearing impairment, visually impaired, with autism, SP-66 - students with motor disability, moderate and mild intellectual disability, autism, multiple disability

ROUTE 2 – 6 children (route about 28 km / day)

A vehicle with seating capacity is required for a minimum of 7 children + 2 seats for the driver and carer

Transport from the designated place to the Special Primary School No. 60 in Katowice, 4 Chrobrego Street (SPS-60) in the School Complex No. 3 in Katowice and Primary School No. 58 with Maria Dąbrowska Integration Classes in Katowice, Al. B. Krzywoustego 11 (SP-58) and return.

Type of disability: SPS-60 - students with autism, Asperger's syndrome and multiple disabilities, SP-58 - students with mild and moderate intellectual disability, motor disability including aphasia, multiple disability, hearing impairment, visually impaired, autism.

Drop-off at 7:45 a.m. to SPS-60 and 8:00 a.m. to SP-58, drop-off from 3:30 p.m.

ROUTE 3 – 4 children (route about 30 km/day)

A vehicle with seating capacity is required for a minimum of 6 children + 2 seats for the driver and guardian

Transport from the designated place to the Special Primary School No. 60 in Katowice, 4 Chrobrego Street (SPS-60) in the School Complex No. 3 in Katowice and the Supporting Education Complex in Świętochłowice, 75 Wojska Polskiego Street (ZEW) and return.

Type of disability: SPS-60 - students with autism, Asperger's syndrome and multiple disabilities, ZEW - students with moderate intellectual disability

Drop-off at 7:45 a.m. to SPS-60 and 8:00 a.m. to ZEW, drop-off at 3:05 p.m. from ZEW and 3:25 p.m. from SPS-60 (or earlier, according to the schedule)

ROUTE 4 – 2 children (route length about 32 km / day)

A vehicle with a number of seats for a minimum of 4 children + 2 seats for the driver and guardian is required.

Transport from the designated place to the John Paul II Special Primary School No. 39 in Katowice, 17 Ścigały Street (SPS-39) and return.

Type of disability: students with mild and moderate intellectual disability, autism, multiple disability

Drop-off at 8:00 a.m., drop-off at 4:00 p.m. (or earlier, according to the schedule)

ROUTE 5 – 4 children (route about 36 km / day)

A vehicle with a number of seats for a minimum of 6 children + 2 seats for the driver and guardian is required.

Transport from the designated place to the Primary School No. 38 in the School and Preschool Complex No. 11 in Katowice, 11 Wantuły Street (SP-38) and Primary School No. 67 with Integration Classes named after the National Education Commission in Katowice, 5 Zielona Street (SP-67) and return.

Type of disability: SP-38 - integration classes for children with mild and moderate intellectual disability, motor disability, autism, Asperger's syndrome, SP-67 - integration classes for visually impaired students, with autism, motor and intellectual disabilities, multiple disabilities.

Delivery at 7:15 to SP-38 and 7:45 to SP-67, drop-off at 15:20 from SP-67 and at 15:40 from SP-38.

ROUTE 6 – 4 children (route about 32 km / day)

A vehicle with a number of seats for a minimum of 6 children + 2 seats for the driver and guardian is required.

Transport from the designated place to the Primary School No. 11 with Tadeusz Kościuszko Integration Classes in Katowice, 16 Nasypowa Street (SP-11) and Primary School No. 38 in the School and Preschool Complex No. 11 in Katowice, 11 Wantuły Street (SP-38) and return.

Type of disability: SP-11 - integration classes for students with mild intellectual disability, hearing loss, visually impaired, Asperger's syndrome, aphasia, SP-38 - integration classes for children with mild and moderate intellectual disability, motor disability, autism, Asperger's syndrome.

Drop-off at 7:45 a.m. to SP-11, 8:00 a.m. to SP-38, drop-off at 4:00 p.m. from SP-38 and 4:25 p.m. from SP-11 (or earlier, according to the schedule)

ROUTE 7 – 2 children (route about 30 km/day)

A vehicle with a number of seats for a minimum of 4 children + 2 seats for the driver and guardian is required.

Transport from a designated place to the School and Preschool Complex for Deaf and Hard of Hearing Children in Katowice, Grażyńskiego 17 (ZSPdDN) and K. K. Baczyński Primary School No. 36 in Katowice, 13 Iłłakowiczówny Street (SP-36) and return.

Type of disability: ZSPdDN - deaf and hard of hearing students.

Drop-off at 7:40 a.m. to ZSPdDN and 8:00 a.m. to SP-36, drop-off from 3:30 p.m. (or earlier, according to the schedule)

ROUTE 8 – 3 children (route about 48 km / day)

A vehicle with a number of seats for a minimum of 5 children + 2 seats for the driver and guardian is required.

Transport from the designated place to the Primary School No. 51 with Integration Classes in Katowice, Przyjazna 7A (SP-51) in the School and Preschool Complex No. 13 in Katowice and the Dr. Maria Trzcińska-Fajfrowska Rehabilitation, Education and Education Center in Katowice, 8 Gościnna Street (OREW) and return.

Type of disability: SP-51 - integration classes for students with mild intellectual disability, motor disability, multiple disabilities, autism, visually impaired and hard of hearing, OREW - students with moderate, severe and profound intellectual disabilities, motor and multiple disabilities

Drop-off at 7:50 a.m. to SP-51 and 8:00 a.m. to OREW, drop-off from 4:00 p.m. (or earlier, according to the schedule)

ROUTE 9 – 3 children (route about 42 km / day)
