The subject of the contract is "Increasing the efficiency and availability of e-services in Centrum Medyczne w Łańcut Sp. z o.o." in accordance with the description and requirements contained in Annex No. 3 to the SWZ under the name "Description of the subject of the contract".
In connection with the implemented project called "Increasing the efficiency and availability of e-services in the Medical Center in Łańcut Sp. z o.o." by "Centrum Medyczne w Łańcut" Sp. z o.o. under No. RPPK.02.01.00-18-0020/20-00 co-financed by the European Regional Development Fund under Priority Axis No. II "Digital Podkarpackie" of the Regional Operational Program of the Podkarpackie Voivodeship for 2014-2020 from project type 7: Projects in the field of production/modernization/development of e-services available as part of the Podkarpackie Medical Information System (PSIM).
1. The subject of the contract is "Increasing the efficiency and availability of e-services in Centrum Medyczne w Łańcut Sp. z o.o." in accordance with the description and requirements contained in Annex No. 3 to the SWZ under the name "Description of the subject of the contract".
In connection with the implemented project called "Increasing the efficiency and availability of e-services in the Medical Center in Łańcut Sp. z o.o." by "Centrum Medyczne w Łańcut" Sp. z o.o. under No. RPPK.02.01.00-18-0020/20-00 co-financed by the European Regional Development Fund under Priority Axis No. II "Digital Podkarpackie" of the Regional Operational Program of the Podkarpackie Voivodeship for 2014-2020 from project type 7: Projects in the field of production/modernization/development of e-services available as part of the Podkarpackie Medical Information System (PSIM).
2. Tenders not containing the full scope of the subject of the contract described in this GTC shall be rejected.
3. The subject of the contract must be delivered to the IT Department of the Ordering Party, implemented and installed in full.
3.1. All supplied:
• Products (understood as an elementary effect of activities/works/deliveries covered by the entire scope of the Subject of the Order performed by the Contractor during the performance of the Agreement in individual stages).
• Components (understood as an integral part of the delivery and implementation of the Subject of the Order, consisting of at least one Product or many Products related to each other)
are subject to design, supply, installation, configuration and implementation services.
3.2. The services of design, installation, configuration and implementation shall be carried out by the Contractor in accordance with the provisions of the OPZ (description of the subject of the contract) in consultation with the Contracting Authority, in accordance with applicable regulations, rules for the implementation of ICT projects and best practices in their implementation.
3.3. The contractor must provide all equipment and components that are necessary for the complete implementation of the integration with PSIM. If, during the integration of the Subject of the Contract, it turns out that any device or element is missing or that the Contractor uses a different way of meeting the requirements than the one included in the SWZ, the lack of which will result in improper cooperation of the supplied hardware and software with the Regional Platform, the Contractor shall provide them at its own expense.
3.4. All equipment supplied:
• They must be brand new, come from the manufacturer's authorized sales channel and represent the model of the current production line. Refurbished, demonstration or post-exhibition equipment is not allowed.
• Devices with a legal defect in terms of equipment origin, technical support and manufacturer's warranty are not allowed.
• The components from which the devices are built must be products of the device manufacturer or be certified by him and the whole must be covered by the manufacturer's warranty.
• Equipment and its components must be labelled in such a way that both the product and the manufacturer can be identified.
• Devices must be delivered to the Ordering Party in the original manufacturer's packaging.
• A set of standard documentation for each device must be provided for
• user in Polish or English in paper or electronic form.
• Devices at the stage of delivery between the manufacturer and the Ordering Party may not be subject to modifications.
