The subject of the contract are successive deliveries of reagents for biochemical tests and reagents for coagulological tests along with the rental of analyzers for the needs of the Independent Public Health Care Team in Myszków. The order is divided into 2 packages (parts). A detailed description of the subject of the contract and the conditions for the performance of the contract shall specify:
- Price form – Annex 1.1A and Annex 1.1B to SWZ,
- draft provisions of the agreement – Annex 2 to the SWZ.
The agreements for the individual packages will be concluded after the end of these proceedings for a period of 36 months.
