The target of the procurement is an image-controlled isocentric linear accelerator for radiotherapy with peripheral devices.

Vaasa Central Hospital's radiotherapy unit has three radiotherapy units: Varian Truebeam "Hoitokone L1" (6MV, 6FFF, 10MV and 15MV), and Varian Truebeam L2 (6MV and 6FFF) and Varian Truebeam L3 (6MV, 10MV and 15MV). The radiation properties of therapy equipment have been matched. At the moment, patients can be exchanged, for example, in the form of a patient. in the event of a failure, from one dental unit to another directly within the framework of the treatment energy. The unit currently treats about a little over a thousand patients annually. The oldest of the therapy devices (Truebeam L3) is now being replaced by a new linear accelerator.

For the functioning of the clinic, reliability, reproducibility, flexibility, efficient use of time and continuity are important. Patients often come from a long distance, so it must be possible to administer radiation therapy appropriately even in the event of a fault and, for example, in the case of radiotherapy. during maintenance. It is therefore absolutely essential that, if necessary, patients can be replaced from the equipment now being procured to another radiotherapy equipment in the clinic and vice versa, without additional work and uncertainty. The system to be procured is also the only possible backup machine for 10 Mv and 15 MV – energetically treated L1 dental unit patients.

The system provided must be able to treat the patient with the corresponding energy from the treatment plan made for the dental unit L1 or L2, and vice versa. It is considered an advantage if the patient and the radiotherapy plan can be directly picked up from the list of another dental unit, and transferred from one dental unit to another and vice versa without additional programs, or systems, or procedures.

For dose planning imaging, a Toshiba (Canon) Aquilion LB CT device is used in the clinic. The breath-paced design description and the design description of deep inspiration breath hold (DIBH) are implemented using the Varian RPM system. Varia's Identify surface detection system is also used in radiotherapy planning descriptions.

The unit uses the Varian Aria (version 16) information and verification system. Dose planning is done for all dental units using Varia's Eclipse dose planning system (version 16). The images and radiotherapy history of all dental units (including the system provided) are stored, etc. information system for radiotherapy servers. Therefore, the patient's treatment information, images and parameters must be automatically stored in the clinic's information system from the system provided after treatment. An offer of a system in which treatment data and/or dental unit images are transferred manually, or only transferred at the end of the treatment period or weekly to the information system, will not be accepted.

The system to be provided must be compatible with, and form a functional whole, with the existing information, design and verification system of the unit. It is considered an advantage if compatibility is realized without additional systems or programs.

The tender shall specify how compatibility will be achieved in the entity to be offered and in each system. In particular, it must be presented how the settings of the dental unit are verified, how the patient can be switched from one dental unit to another during treatment, how the treatment plan is transferred from dose planning to the dental unit, and how the pictures, treatment parameters and treatment history taken with the dental unit are transferred to the existing Aria system.

The system offered must be Dicom RT compliant. The equipment offered must be new and unused.

The supplier may only offer a device of the kind installed in a hospital in Europe. In the tender, the supplier must present a list of European reference clinics where the system offered is in use in the same environment (Aria/Eclipse information and dose planning system, and at least one Truebeam therapy device) as in Vaasa now. If necessary, at least one reference clinic shall be visited in February-March -23 to establish how the compatibility of the offered device with the information system (Aria/Eclipse 16/Truebeam) has been implemented. The provider is responsible for the travel and accommodation costs of the visit for three people. Communication in the reference clinic in English or Finnish.

The components of the offer shall be existing systems that have been proven to work in clinical use.

The accelerator shall be capable of administering Intensity Modulated Radiation Therapy IMRT as Image Guided Radiation Therapy IGRT, as well as Intensity Modulated Arc Therapy IMAT. The device must be capable of imaging cone beam CT images (Cone Beam CT, CBCT). Similarly, the device must be capable of imaging control imaging during arc radiotherapy with stop automation (e.g. gold grain-targeted prostate short (7 fractions) radiotherapy). Similarly, the device must be capable of administering arc radiation therapy so that the Multi-Leaf Collimator (MLC) is bounded by the outer edge of the target area throughout the arc.

The system offered will also provide stereotactic radiotherapy for metastases, which is why it must have so-called metastasy radiotherapy. 6D table.

It must be possible to plan radiotherapy for the therapy equipment offered in its entirety and for all treatment techniques in the clinic. with a dose planning system. Tenders for a system where this is not possible will not be accepted.

The new accelerator is placed in the same treatment room, from where the old device (Varian Truebeam) is removed.

An open procedure is used as a procurement procedure.

The procurement complies with the Act on Public Procurement (1397/2016).
