1. The subject of this order is the DELIVERY OF DISPOSABLE EQUIPMENT FOR THE VASCULAR SURGERY DEPARTMENT OF THE PROVINCIAL SPECIALIST HOSPITAL OF THE BLESSED VIRGIN MARY IN CZĘSTOCHOWA.
2. The subject of the contract includes 3 parts. The description of the subject of the contract has been included in the assortment and price forms constituting Appendix No. 2 to the Terms of Reference. Other terms and conditions concerning the performance of the contract are set out in Appendix No. 7 to the Terms of Reference, which is a model agreement.
