The contractor's portfolio must include the following services:

- Support, advice and care of insured persons with heart failure of NYHA stages I-IV

- The contractor's range of services includes assistance and information on the use of standard care (DMP programs, telemonitoring in standard care, regular use of general and specialist care). The further development of the DMP programs and the standard care must be taken into account on an ongoing basis.
