1. The subject of the contract is the delivery consisting of the successive delivery by the Contractor to the office of the Ordering Party of consumable materials to the Hemodynamics Laboratory and the lease and service of the system (equipment) for electronic and mechanical endovascular ultrasound (IVUS) together with Physiological measurement guides (FFR).

2. A detailed description of the subject matter of the contract in terms of consumable materials and system leases is set out in Annex 2 to SWZ, assortment and price form.

1. FFR/iFR guide measuring the relevance of coronary artery stenosis without the need for hyperemia, min. 185 cm long and 0.014" diameter — pieces: 217;

2. IVUS catheters — electronic ultrasonic resolution head 20 MHz (+/- 2 MHz), catheter with a working length of min. 150 cm, with the possibility of insertion into the lumen of a vessel using a catheter with a diameter of 5F, with a low entry profile of not more than 0,019 French — pieces: 37;

3. Mechanical IVUS catheters — ultrasonic mechanical head with a resolution of 45 MHz (+/- 2 MHz), catheter with a minimum operating length of 135 cm, with the possibility of insertion into the lumen of a vessel using a catheter with a diameter of min. 5F, with a reusable winch — pieces: 28;

4. lease of the system (equipment) for electronic and mechanical endovascular ultrasound (IVUS) with physiological measurement guides (FFR) for a period of 12 months.
