To provide specialist home care services for adults resident within the City of Newcastle who have complex specialist needs including physical disabilities, sensory support, cognitive impairments and/or mental health needs.
To provide specialist home care services supporting people with physical disabilities will cover a broad range of long term conditions. These conditions can be fluctuating, progressive and in some cases life-limiting.
To provide specialist home care services supporting people with sensory disabilities will be required to provide an individualised service to people who have sensory support needs.
To provide specialist home care services supporting people with a cognitive impairment, or dementia related illness, and there should be complexity in delivering this service.
To provide specialist home care services supporting people who have fluctuating conditions and fluctuating levels of independence and/or be those who suffer negative effects of their mental health condition.
The support provided should be person centred, holistic and non-stigmatising. The Service Provider should be skilled in utilising an approach which is strength based, focusing on assets, independence, recovery and social inclusion with no wrong door.
