1. The subject of the contract is the provision of services in the field of preparation and delivery of full-day food for the Ordering Party's patients.
Package No. 1
a. preparing full-board meals for the patients of the Poviat Hospital in Mikołów at ul. Waryńskiego 2 and for patients of the Care and Care Center (ZPO) in Łaziska Średni at ul. Staszica 4,
b. delivery of meals to the Nutrition Department at the hospital and the point of collection of meals at the ZPO by means of the Contractor's means of transport (with one or two cars),
c. unloading containers with meals from the car and transporting the Contractor's cart to the indicated place,
d. collection of empty containers after meals and their washing and disinfection in the Contractor's premises
Package No. 2
Preparation and delivery of breakfast (approx. 10 breakfasts throughout the duration of the contract) as described in Annex 1 point II.

Package No. 1
a. preparing full-board meals for the patients of the Poviat Hospital in Mikołów at ul. Waryńskiego 2 and for patients of the Care and Care Center (ZPO) in Łaziska Średni at ul. Staszica 4,

2. In connection with the specifics of the subject of the order, the Employer pursuant to art. 29 paragraph 3a. Public Procurement Law Act requires that persons involved in the implementation of the subject of the contract as described in the Employer contained in Annex 1, were employed by the Contractor or subcontractor under an employment contract throughout the duration of the contract. In the event of termination of employment of the persons referred to above before the end of the contract period, the Contractor or Subcontractor shall be obliged to promptly assign another person employed in the required form to this place.
3. At the latest on the day of signing the contract, the Contractor shall submit to the Employer a name list of persons employed by the Contractor or Subcontractor under an employment contract.
4. The Contractor shall attach to each invoice his subcontractor's declaration of employment on the basis of an employment contract of persons performing the service for the Employer. This statement should include, in particular: the exact specification of the entity submitting the statement, the date of submitting the statement, an indication that the services provided to the Employer are performed by persons employed under an employment contract together with an indication of the number of these persons and the signature of the person authorized to make the statement on behalf of the Contractor or subcontractors;
5. Terms of payment - payment will be made within 30 days from the date of invoice, by bank transfer to the Contractor's account indicated on the invoice.
