The subject of the contract includes transport – delivery by "walking floor" cars of waste with the code 19 12 10 (substitute fuel) and waste with the code 16 01 03 (used tires) from the Municipal Installation – Waste Utilization Center of the Lusatian Municipalities in Lubań at 1 Bazaltowa Street to Górażdże Cement S.A, 47-316 Chorula, 1 Cementowa Street.

Object range of order

a) The scope of the contract waste with the code 19 12 10 (substitute fuel) includes:

- Substitution of a "walking floor" car to the Municipal Installation in Lubań at 1 Bazaltowa Street in order to be loaded by the Ordering Party with waste code 19 12 10 (about 18 Mg each time; approx. 90 m3) to the Municipal Installation in Lubań at 1 Bazaltowa Street in order to be loaded by the Ordering Party with waste code 19 12 10 (about 18 Mg each time; approx. 90 m3);

- Transport of cargo in accordance with environmental requirements to the unloading point in the Cement Plant Górażdże Cement S.A. in Chorula at 1 Cementowa Street within the time limit set as the date of unloading;

- Unloading the cargo in accordance with the requirements of Górażdże Cement S.A. constituting an attachment to the SWZ entitled "Car unloading instructions";

The contracting authority assumes, m.in:

- Transport and unloading of cargo (19 12 10) on the route: Lubań Bazaltowa 1 - Chorula Cementowa 1, 7 days a week, 24 hours a day with transport also on holidays in the number of trips resulting from the demand of Górażdże Cement S.A.;

- The contracting authority provides for at least two transports per day with the admission of up to three or four transports per day;

- The date of each unloading is set at the end of each week (Friday) as part of the so-called notification carried out by the Ordering Party at the Recipient of substitute fuel. The Ordering Party reserves the right to increase the number of trips, especially on off-schedule holidays;

- Each time before setting the dates of unloading as part of the so-called notification carried out by the Ordering Party at the Recipient of substitute fuel (Thursday of each week), the Contractor shall provide the Ordering Party: registration numbers of cars and semi-trailers to transport fuel in the notified week and the name and surname of drivers servicing the above-mentioned cars on a given day and contact numbers of drivers (telephone number) in accordance with the "Register" attached to the SWZ and all company data transporting necessary to issue a KPO in the BDO system;

- The Ordering Party reserves the right to order transport after the date of notification;

- The contractor is always obliged to substitute the car sufficiently in advance in order to load it, however, this time may not cause the replacement fuel to be kept in loading without its transport (e.g. unloading has been scheduled for 15:00, loading will take place no earlier than 10:00, followed directly by transport).

(b) Waste code 16 01 03 (used tyres):

- Substitution of a walking floor car to the Municipal Installation in Lubań at 1 Bazaltowa Street in order to be loaded by the Ordering Party to the Ordering Party with waste code 16 01 01 (about 7 Mg each time; approx. 90 m3) to the Municipal Installation in Lubań at 1 Bazaltowa Street in order to be loaded by the Ordering Party with waste code 16 01 03 (about 7 Mg each time; approx. 90 m3).

- Transport of cargo in accordance with environmental requirements to the unloading point in the Cement Plant Górażdże Cement S.A. in Chorula at 1 Cementowa Street, on the date set as the date of unloading;

- Unloading the cargo in accordance with the requirements of Górażdże Cement S.A. constituting an attachment to the SWZ entitled "Car unloading instructions";
