Maintenance on public roads in Częstochowa (within the administrative boundaries of the city) in technical condition of vertical signs and traffic safety devices in the years 2019-2021:

- Performing device status reviews,

- Reporting faults to the Employer and agreeing on the scope of services to be performed,

- Immediate notification of the Employer about the removal of a failure,

- Preparation and submission of the book of measurements,

- Emergency road marking and protection

The awarding entity requires a minimum 24-month warranty and a maximum 60-month warranty for the subject of the contract.

The subject of the contract is to maintain vertical signs and road safety equipment.

The Awarding Entity specifies the following requirements regarding the employment by the Contractor or Subcontractor of persons performing the tasks indicated by the Awarding Entity in the field of contract performance under an employment contract:

1) In the scope of application of art. 29. paragraph 3a, the Employer requires employment by the Contractor or Subcontractor, in the manner specified in art. 22 § 1 of the Act of 26.6.1974 - Labor Code of persons performing the following activities: related to the maintenance, renewal of vertical, horizontal marking and road safety devices.

2) During the performance of the contract, the contracting authority is entitled to perform inspection activities against the contractor regarding the fulfillment by the contractor or subcontractor of the employment requirement under the employment contract of the persons performing the activities specified in point 1. The contracting authority is entitled in particular to:

a) request statements and documents regarding confirmation of compliance with the abovementioned requirements and assessing them,

b) demand explanations in case of doubts as to confirmation of compliance with the abovementioned requirements

c) carry out on-site inspections.

3) During the performance of the contract, the contractor will submit to the contracting authority the evidence indicated below to confirm the fulfillment of the employment requirement by the contractor or subcontractor of persons performing the activities specified in point 1 during the performance of the contract at the time specified in the call,

- declaration of the contractor or subcontractor about employment on the basis of an employment contract of persons performing activities to which the contracting authority refers. This statement should include

In particular: the exact specification of the entity submitting the declaration, the date of submission of the declaration, an indication that the activities covered by the call are performed by persons employed under an employment contract together with an indication of the number of those persons, names and surnames of those persons, type of employment contract and full-time employment and signature of the person entitled to make a statement on behalf of the contractor or subcontractor;

- a copy of the contract (s) of the persons performing activities referred to in the aforementioned contract certified by the contractor or subcontractor as appropriate. declaration of the contractor or subcontractor (together with the document regulating the scope of duties, if drawn up). A copy of the contract (s) should be anonymized in a manner that protects employees' personal data in accordance with the provisions of the Act of 29.8.1997.

On the protection of personal data (i.e. in particular without addresses, PESEL number of employees. The employment contract may also contain other data that is subject to anonymization. Each contract should be analyzed by the applicant in terms of the provisions of the Act of 29.8.1997 on data protection personal; the scope of the contract anonymization must be in accordance with the provisions of the above Act.) The name of the employee is not subject to anonymization. Information such as: date of the contract, type of employment contract and full-time employment should be identifiable;

- a certificate from the competent ZUS branch confirming the payment of social and health insurance contributions by the contractor or subcontractor under employment contracts for the last accounting period;

- a copy of the proof confirming the employer's registration for insurance, certified by the contractor or subcontractor, certified as true to the original, anonymized in a manner ensuring protection of employees' personal data, in accordance with the provisions of the Act of 29.8.1997 on the protection of personal data. The employee's first and last name are not anonymous.
